Prisoners: Self-harm

Question
To ask Her Majesty’s Government, further to the Written Answer by Lord McNally on 23 June (WA 349), whether disciplinary action was taken following the report into the incident of self-harm at HMP Woodhill on 18 April commissioned by the Governor.
Answer

The investigation into the incident was completed on 5 July 2011. The governor concluded, in the light of the findings of the investigation, that no grounds existed on which to justify disciplinary action. The investigation made a number of recommendations and these are being taken forward by the governor.
