To establish an independent inquiry into death of Germain Alexander who died in Kings College hospital on Wed 6 Dec to instruct governor of Brixton prison & Commissioner of Police for Metropolis to set up an inquiry to establish how Mr Alexander incurred serious injuries on 5 & 6 Dec while in police & prison custody.
