I clarify again that the report would be the product of the local investigation carried out by the trust. I would encourage trusts to go further than simply laying out the facts. If they wanted to make an apology to the patient at that stage or explain what process changes have been made to prevent the same thing happening again, it would be right and proper for them to do that. I would encourage them to do so, rather than delivering a mechanistic report dealing solely with the facts of the case.
