Mr. Chope: To ask the Secretary of State for Health what steps he is taking to ensure that the waiting list for access to the INPUT pain management clinic at Guys and St. Thomas's hospital is reduced; and when he will respond to the letter from Mrs. Jackson of Christchurch dated 5 December 2003 on this subject. [150536] Mr. Hutton: Officials responded to Mrs. Jackson's letter of 5 December 2003 regarding the INPUT pain clinic on the 13 January 2004. I am assured by Guy's and St. Thomas' Hospitals National Health Service Trust that they have taken steps to reduce waiting times for the INPUT pain clinic: Extra clinic sessions have been added. All referrals have to be validated by the consultant. The non-general practitioner referral waiting list is currently being validated to ensure that patients still wish to attend for assessment. Improvements in the administration system mean it now provides better information about clinic slot availability and facilitates the offer of vacant slots to non-GP referral patients. Staff are now able to compile reports in order to manage clinics more effectively. A review of the systems for sending out-patient appointments has taken place and a reminder system will be introduced to minimise the number of appointment slots, which might otherwise be wasted if patients did not attend. To increase psychologist out-patient capacity, the weekly timetable for all psychologists is currently being reviewed, in order to identify how their workloads can be altered to allow more outpatient assessment time. The trust also plans to employ a service improvement facilitator to help improve the service provided by the clinic.

[140773] Mr. Hutton: The Clinical Standards Advisory Group report showed that many national health service trusts had developed excellent services, but in some places more needed to be done to drive up standards. The report made recommendations to commissioners and trusts to review local provision of pain services, and how these might be improved. The Department welcomed the report, and expects commissioners and trusts to take these recommendations into account when they plan their service so that variations in provision is reduced and links with primary care trusts improved. The Department encourages openness in the use of NHS performance information and welcomes independent scrutiny of this information, such as the Dr. Foster and Pain Society report on "Adult Chronic Pain Management Services in the UK". We are committed to making NHS performance information widely available to patients, carers and the public in an accessible and readily understandable format. The Department has no plans to review the provision of pain services in the NHS or to reissue the Clinical Standards Advisory Group Report. The quality of the service should be monitored locally through the trust, primary care trust commissioners and strategic health authorities to ensure it is provided to meet the needs of the local population.

Mr. Wray: To ask the Secretary of State for Health what steps he is taking to assist people living with persistent pain; what pain relief drugs are available; and if he will make a statement on the (a) effectiveness of such drugs and (b) problems with their side effects. [128078] Mr. Hutton: Pain management is an important component of most patients' care. The Clinical Standards Advisory Group (CSAG) report on pain services, published in 2000, highlighted variations in access to pain services throughout the country. The report made recommendations to National Health Service acute trusts and commissioners on how pain services should be delivered in order to reduce the variations to access. It recommended that primary care trust commissioners should review local provision of pain services, looking particularly at the provision of more specialised treatments on a networked basis. In this review, account should be taken of the needs of both adults and children, and include patients with acute pain resulting from sudden illness or accident, as well as post- operative pain and chronic pain. Trusts should agree with commissioners the services that are appropriate to meet local needs. All medicines are assessed before licensing by the Medicines and Healthcare Products Regulatory Agency (MHRA), with independent expert advice from the Committee on Safety of Medicines (CSM). A licence will not be granted unless the MHRA and the CSM are satisfied that the medicine meets appropriate standards of safety, quality and efficacy at the time of licensing. Details of the different classes of medicines used in the treatment of persistent pain and their side effects are contained within the British National Formulary (section 4.7, page 208, BNF 45, March 2003), a copy of which has been placed in the Library.

[126349] Mr. Hutton: Information is not available on the number of posts or vacancies for staff specialising in pain relief. Pain relief is a sub-specialty of anaesthetics. Information on whole-time equivalent staff in post for consultants, registrars and senior house officers in anaesthetics is shown in the table. Information on the number of whole-time equivalent posts in national health service trusts is not collected centrally. Information on vacancies lasting three months or more is collected in the NHS vacancy survey which has been collected in March each year since 1999. Information specifically for the anaesthetics (inc. intensive care) specialty has only been collected since 2000. There is no comparable information on nurses or allied health professionals. Between 1997 and 2002, the number of whole-time equivalent consultants working in anaesthetics (inc. intensive care) have increased by 40 per cent., registrars by 10 per cent., and senior house officers by 14 per cent. It made recommendations to national health service acute trusts and commissioners on how pain services should be delivered in order to reduce variations to access. In this review, account should be taken of the needs of both adults and children, and include patients with acute pain resulting from sudden illness or accident, as well as post-operative pain and chronic pain. Trusts should also agree with commissioners the services and resources which are appropriate to meet local needs. The Department of Health does not collect information on those trusts which do not provide a chronic pain service. The CSAG report showed that, in April 2000, chronic pain services existed in 215 acute NHS trusts, which amounts to 86 per cent. of acute trusts in the United Kingdom.

Dr. Kumar: To ask the Secretary of State for Health what the cost of back pain was to the NHS in the last year for which figures are available; what research is being conducted into the management of such pain; how many treatment centres are available in (a) England and (b) each Government Office region; what the average referral time is for patients diagnosed with back pain and related conditions; what plans there are to provide additional resources to tackle this condition; and if he will make a statement. [121643] Mr. Hutton: Comprehensive information is not held centrally on the cost of back pain to the national health service or on referral times for patients diagnosed with back pain. At national level, the NHS research and development health technology assessment programme is currently supporting two research projects on the management of back pain covering, respectively, whether early imaging influences management and improves outcome in patients with low back pain, and the longer term clinical and economic benefits of offering acupuncture to patients with chronic low back pain. Treatment is available very widely. It is for primary care trusts to determine what level of resources to devote to the treatment of different conditions in their localities. The Government are currently exploring a number of ways to improve access to occupational health services and vocational rehabilitation for all workers, including those suffering from back pain, through NHS Plus and the Welfare to Work reforms. NHS Plus, launched in November 2001, is currently providing access to local occupational health services to small and medium sized businesses through 107 providers (NHS trusts) across the United Kingdom; while, the NHS "Back in Work" campaign aims to reduce the causes of back pain and the incidents that lead to back pain in the NHS. In addition, the new Job Retention and Rehabilitation pilots, launched on 1 April 2003, and the Incapacity Benefit reforms, announced in the Government's Green Paper in November 2002, "Pathways to Work", are offering work-focused interventions for those with health problems, including back pain, to help people return to work.
