Paul Goggins: The numbers and percentages of individuals who self-harmed or apparently took their own lives in prisons (1999-2003),
are included in the following tables. While the new self-harm reporting system which started in 2003 has improved the consistency
with which such incidents are reported, and improved the detail and quality of the information collected, it makes meaningful
comparisons across time more difficult.

Angela Smith: Information on the total number of episodes of self-harm is not available. However, information is available on the
number of episodes of self-harm resulting in admission to acute hospitals in Northern Ireland.
The following table provides figures on the number of admissions to hospitals in each trust area in Northern Ireland as a result of
self-harm for each year from 2000-01 to 2002-03 (the latest year for which the data is available).

Dr. Vis: To ask the Secretary of State for the Home Department how many incidents of (a) self-injury and (b) attempted

Paul Goggins: Suicidal intent is difficult to establish. Data recording incidents of `attempted suicide' are subsumed within those
for self-harm, which cover all acts of self-injury however serious.
A new form for reporting self-harm was introduced across the prisons estate in December 2002. This has improved the consistency with
which self-harm incidents are reported and, by broadening the range of questions that are asked, improved the detail and quality of
the information collected. Prior to 2003, data on the location where the self-harm incident occurred were not collated centrally.
The following table provides the information requested with respect to 2003 only.

segregation. Note: The table records number of incidents, not number of individuals. Establishments showing particularly high
numbers reflect individual cases of repetitive self-harm.
A number of intervention strategies have been introduced into prisons, young offender and juvenile establishments for people who
self-harm. These include counselling, support groups, and specialised psychological interventions. While evaluation of these
interventions is, at present, very limited, evidence from research in non-prison settings suggests that male and female adolescents
who self-harm repeatedly may benefit from group therapy. A network of establishments has been set up to develop interventions,
facilitate evaluation and share good practice. Guidance to staff on managing people who self- harm has also been circulated to
establishments.

Paul Goggins: The information requested about self- inflicted deaths and self-harm is provided in the following table. Suicidal
intent is not easy to establish. The reasons why people self-injure are highly complex, and it is difficult to distinguish between
acts of self-harm that were attempts at ending life, and those that occurred for other reasons. It is not possible, therefore, for
the Prison Service to determine from the general figures recording incidents of self-harm those acts that could be described as
attempts at suicide. The figures for self-harm in the table therefore include all acts of self- harm, however serious.

self- harm in 2003 may result from the change in reporting procedures rather than reflect an actual increase in incidence of

In 2004 to 16 June, there have been two apparently self-inflicted deaths at Holloway; the number of incidents of self-harm is
not yet available.
My right hon. Friend the Home Secretary, visited Holloway in April 2004, and I visited the establishment in October 2003. We both
had the opportunity to talk with the Governor, with a number of prison staff from a variety of disciplines, and with groups of
prisoners. It was clear from our wide-ranging discussions that Holloway is dealing with a large number of vulnerable women, many of
whom enter custody already struggling to cope with a wide range of difficult issues that increase their likelihood of suicide and
self-harm. These include mental health, drug and alcohol problems; histories of abuse, troubled relationships and social exclusion;
as well as past experiences of self-harm.
Both my right hon. Friend the Home Secretary and I were impressed with Holloway's First Night in Custody Project, which focuses on
the needs of women at the vulnerable stage of entry into prison. The Project was set up in October 2000 and is run by the Prisoner
Advice and Care Trust. It employs a full-time project worker and two link workers to help identify women at risk of self-harm or
suicide. The project sees over 100 prisoners a month and makes referrals to services in and outside the prison on housing needs,
mental health problems, drug or alcohol problems.
Holloway is also one of the three prisons currently piloting Dialectical Behaviour Therapy (DBT), an innovative programme developed
for women with `borderline personality disorder' who also self-harm or engage in suicidal behaviours.
20 safer cells are to be installed at Holloway by October 2005. Safer cells, which are manufactured and installed to make the
attachment of ligatures very difficult, have been found effective in preventing impulsive suicide attempts.
Across the prisons estate, women prisoners are to benefit from a specifically targeted and separate suicide prevention and self-harm
management strategy being developed for them. This draws upon specialist knowledge of suicidal behaviour among women prisoners, and
builds upon a number of interventions including: individual crisis counselling for women prisoners who self-harm; the continued
development and evaluation of DBT; investment and planning to ensure progress on the detoxification strategy in women's prisons; and
the introduction of a new training pack for all staff working with women in custody, which includes a module on the health and
well-being of women prisoners. £1 million from the Department of Health has been allocated to women's prisons to be spent on
the recruitment of psychiatric nurses.

Paul Goggins: In the calendar year 2003, there were 851 recorded acts of self-harm involving prisoners at HMP Holloway.
Suicidal intent is not easy to establish. The reasons why people self-injure are highly complex, and it is difficult to distinguish
between acts of self-harm that were attempts at ending life, and those that occurred for other reasons. It is not possible,
therefore, for the Prison Service to determine from the general figures recording incidents of self-harm those acts that could be
described as attempts at suicide. The figure above therefore includes all acts of self-harm, however serious.
Women prisoners are to benefit from a specifically targeted and separate suicide prevention and self-harm management strategy being
developed for them. This builds upon a number of interventions including: individual crisis counselling for women prisoners who
self-harm; the continued development and evaluation of Dialectical Behaviour Therapy, which is currently being trialled at Durham,
Bullwood Hall and Holloway prisons; investment and planning to ensure progress on the detoxification strategy in women's prisons;
and the introduction of a new training pack for all staff working with women in custody, which includes a module on the health and
well-being of women prisoners. £1 million from the Department of Health has been allocated to women's prisons to be spent on
the recruitment of psychiatric nurses.

Paul Goggins: I am not aware of any case where a juvenile (aged 15 to 17 years) has used safer clothing to self-harm. However, such
data are not collated centrally, and examination of each individual incident would involve disproportionate cost.
Prison Service policy is that normal clothing must not be removed from at-risk prisoners as a matter of course, but as necessary for
the immediate safety of the prisoner, and then for the shortest time possible.

Paul Goggins: [holding answer 11 May 2004]: There were seven reported incidents of self-harm in the Segregation Unit at Stoke
Heath young offender institution involving juveniles during 2003, and one so far reported in 2004.
A new form for reporting self-harm was introduced across the prison estate in December 2002. This has improved the consistency with
which self-harm incidents are reported and, by broadening the range of questions that are asked, improved the detail and quality of
the information collected. Prior to 2003, data on the location in the prison where the self-harm incident occurred were not collated
centrally.
Suicidal intent is not easy to establish. The reasons why people self-injure are highly complex, and it is often difficult to
distinguish between acts of self-harm that were attempts at ending life, and those which occurred for other reasons. It is not
possible, therefore, for the Prison Service to extrapolate from the data recording incidents of self-harm those acts that may have
been attempts at suicide.
A number of intervention strategies have been introduced into prisons, young offender and juvenile establishments for people who
self-harm. These include counselling, support groups, and specialised psychological interventions. While evaluation of these
interventions is, at present, very limited, evidence from research in non-prison settings suggests that male and female adolescents
who self-harm repeatedly may benefit from group therapy. A network of establishments has been set up to develop interventions,
facilitate evaluation and share good practice. Guidance to staff on managing people who self- harm has also been circulated to
establishments.

developing a separate but related self-harm management strategy.

Mr. Caplin: It is notoriously difficult to obtain reliable and consistent data about incidents of self-harm, because of problems of
definition and identification, uncertainty about intention, patient confidentiality and the variety and inconsistency of reporting.
As I indicated previously, more work is continuing to try to improve the collection of information.

Mr. Caplin: The Department is considering whether it is feasible to collect information on incidents of self- harm that would be
reliable and consistent between the three Services. The work in hand is looking at the identification and recording of such
incidents, reporting systems, analysis and data protection. This is, however, a complex area and an early conclusion is unlikely.

Ms Rosie Winterton: The National Institute for Clinical Excellence is currently preparing guidelines for the management of self-harm
in accident and emergency departments. This includes the services provided to children and young people. Once published, the
National Institute for Mental Health in England (NIMHE) will support local services in establishing procedures and services for
people presenting at accident and emergency with self-harm.
A risk assessment training package (STORM) focusing on self-harm is being piloted in two areas and in five prisons.
NIMHE is supporting local mental health services in implementing the 12 points to a safer service-- developed from the work of the
National Confidential Inquiry into Suicide and Homicide--which includes recommendations for patients who self-harm.

Charles Hendry: To ask the Secretary of State for Health what research his Department has commissioned into (a) suicide and
(b) deliberate self- harm in young people; and if he will make a statement on the findings of the research. [161082]
Ms Rosie Winterton: A national collaborative is being established with Department of Health funding for the monitoring of deliberate
self-harm (DSH), including that of young people. Through this monitoring it will be possible to estimate the number of suicides in
the year following DSH and thereby inform suicide and deliberate self-harm strategies.
The Department has recently funded a study of coroners' records covering different methods of suicide across the whole age range.
The Department also funds the Oxford Monitoring System for Attempted Suicide. Its aims are to provide comprehensive monitoring of
long-term trends in DSH; the identification of information (causes, risk factors and outcome) relevant to service provision for DSH
patients; an evaluation of suicide prevention strategies; and specific research projects on DSH.
In addition, mental health promotion pilots targeted at young men are being established in three areas, which, if successfully
evaluated, will become part of the National Institute for Mental Health in England's mental health promotion work.
