I hope that the Minister will accept that I recognise the difficulties that she faces in controlling the flow, given what she said. Has she considered the British Medical Association's argument that there could be some form of restitution to countries? We know how many nurses are coming in from these countries, albeit not through active recruitment by the NHS, so could we not propose a scheme through which NHS workers could be funded on NHS salaries to volunteer—I think that they would be willing to do so—to go out in equal numbers to compensate those countries and provide their skills?

An active programme of work is under way to do just that. The hon. Gentleman is absolutely right that it is important for us to help developing countries to develop their health care systems in exactly that way. Much of the work is carried out through the Department for International Development, which recently put about £580 million into assisting the development of health care systems. DFID has worked jointly with the Department of Health and volunteers from the NHS. Some 40 per cent. of NHS trusts now have international links to allow volunteers to go to other countries and join in with Government-funded programmes, which are proving to be extremely successful.

The Prime Minister is of course right, but does he realise that the UK is poaching thousands of doctors and nurses from sub-Saharan Africa without any form of restitution, such as specific funding or sending our own manpower back? Indeed, the Government are telling British doctors not to treat the very small number of African patients with HIV who have no status in this country and are forcing them to go back to countries in which they have very little chance of treatment, and thus, most likely, to their death. How can the Prime Minister claim moral leadership on the matter with G8 leaders when our current policy and practice appears to be to steal the doctors and reject the patients? Will he change his policy—

There is no policy to steal doctors and nurses from developing countries. There is a code of practice to which we adhere that ensures that we do not do such a thing. If many of the people who come to this country were not coming here, I am afraid that they would not be staying in their own countries—that is the problem—because they would simply be going elsewhere. On the hon. Gentleman's point about turning away some people with HIV/AIDS from the national health service here, he will know that there has been a lot of concern about people coming here to be treated, so we have to be careful about that as well.
It would be grossly unfair to imply, as the hon. Gentleman did, that this country has not been taking a lead on HIV/AIDS. We have made huge investment in the global health fund. Nearly 1 million people now receive AIDS treatment, in part as a result of the work done by this country, and we are leading the international commitment to get action on both drugs and health clinics and the staffing that the poorest countries need. However, that will not be helped, frankly, by refusing to take anyone from the continent of Africa in circumstances in which those people have already made the decision to leave their own countries.

Dr. Evan Harris: To ask the Secretary of State for the Home Department if he will reform the immigration procedures for doctors from
overseas to enable those who wish to work and train in the UK to do so more quickly. [179811]
Mr. Browne: There are currently no plans to change procedures specifically for the admission to the UK of overseas doctors. Doctors
from outside the European economic area who wish to work in the UK normally require a work permit or permission to work through the
Highly Skilled Migrant Programme (HSMP).
All doctor posts are deemed in shortage which means Work Permit (UK) applies a simplified work permit application procedure for
these posts. In the period April 2003 to March 2004 4,377 work permits were approved for a range of posts requiring doctors.
The HSMP scheme is a points based scheme designed to allow individuals with exceptional personal skill and experience to come to or
remain in the UK to seek and take up work. In the period April 2003 to March 2004 699 HSMP applications were approved for a range of
posts requiring doctors.
In addition, the immigration rules also make separate provision for postgraduate doctors to come to the United Kingdom to continue
their training without requiring a work permit.

Dr. Evan Harris: To ask the Secretary of State for the Home Department what steps he is taking to reduce the (a) cost to and
(b) administrative burden on overseas doctors in training of renewing their permit-free postgraduate training status to
enable them to work in the NHS. [179809]
Mr. Browne: Overseas doctors here for postgraduate training do not require work permits but must meet the requirements of the
Immigration Rules for leave to remain in this capacity. Charging for general leave to remain applications was introduced on
1 August 2003 and individual applicants must pay the requisite fee. The fee is £155 for a postal application and £250 for
personal callers who wish to apply at one of the four Public Enquiry Offices.
Fees are set by Treasury rules to recover the full administrative cost entailed in considering applications and no more. We will be
reviewing the fee annually and as such consideration will be given to improving the way the charging system is operated and the
feasibility of differential charges. We have announced that there would be consultation before any more changes to the fee are made,
and the consultation is planned for this autumn.
Since the introduction of charges on 1 August 2003, we have been completing 70 per cent. of postal applications within
three weeks of receiving them in the Immigration and Nationality Directorate (IND) and are continuing to do this. We may not be able
to complete applications within three weeks of receipt if they need further documents, inquiries or an interview, or if they are
complex. We should deal with these within 13 weeks at most. If applicants use the right form, pay the correct fee, provide all the
required documents and photographs, and sign and date the form, it will help the chances of their application being completed
quickly.

Beverley Hughes: The highly skilled migrant programme was launched on 28 January 2002. As at 30 November 2003 a total of 8,628
applications have been received. From 28 January to 31 December 2002-- 1,194 applications were approved. From 1 January to 30
November 2003--3,864 applications have been approved.
Current management information limitations do not, at present, provide a breakdown in the format requested. However, up to 30 August
2003 the number of applications received for the top ten nationalities were as follows: India--832 United States of America--751

Mr. Hutton: National health service trusts are responsible for their own recruitment and no data on the number and costs of their
advertisements in the foreign media are held centrally.
Since 1 May 1997 the following annual costs for advertising in foreign media have been incurred by the Department to recruit

Mr. Hutton: The Nursing and Midwifery Council registers all nurses who work in the United Kingdom and hold statistics for the number
of overseas nurses who are accepted onto their register.
