Jackie Ballard: To ask the Secretary of State for the Home Department (1) how many prisoners hanged themselves in 1999; [137801] (2) what operational lessons have been learned, and what changes have been made, following the deaths in HMP Leicester in December 1999; and if he will make a statement; [137721] (3) how many prisoners suffered permanent damage as a result of an attempted suicide in 1999; [137802] (4) what was the elapsed time between estimated time of death and discovery of the body for each prisoner who committed suicide in 1998 and 1999. [137800] Mr. Boateng: 86 of the 91 self-inflicted deaths in custody in 1999 were by hanging. There is no reliable information about attempted suicides as at present no distinction is made between forms of self-harm. Long term or permanent physical or psychological damage can result from a variety of forms of self-harm, including substance abuse. There were 173 self-inflicted deaths in 1998 and 1999, and full information about the timing of these deaths in relation to the discovery of the bodies could only be obtained at disproportionate cost. Of 40--the first 20 in both years--cases examined, 22 inquest reports contain no record of estimated time of death. Of the remainder, nine deaths are estimated to have occurred between eight minutes and six hours 25 minutes before discovery of the body and nine deaths between 15 minutes and 78 days afterwards. There have been six self-inflicted deaths in Leicester prison in the last year, including two in December 1999. Internal inquiries are complete in respect of five of the six and have identified no failures of basic procedures or apparent links between the six deaths. Five Coroners' inquests are awaited. Since January 2000, the prison's care suite has been refurbished, is permanently manned and in regular use. Two crisis suites are being introduced. Leicester's good and active Listener scheme has been expanded. Improved accommodation will be provided for new receptions. The continuing rise in prisoner suicides, particularly in local prisons like Leicester, has prompted a major review of the Prison Service's work on suicide prevention. There will be a focus on local prisons like Leicester because they have high proportions of unconvicted prisoners and large throughputs and disproportionate numbers of their populations at the most risk of suicide or self-harm.

Mr. Simon Hughes: To ask the Secretary of State for the Home Department what assessment has been made of the operation and success of each of the secure training centres since opening; for each of the centres (a) what percentage of children breached their orders, (b) what percentage of children re-offended during the period of their order, (c) what percentage of children has been reconvicted during the period of their order, (d) what assessment has been made of the provision of education and training, (e) what percentage of staff has experience of working with children, (f) how many and what percentage of the total number of staff have resigned since each centre opened, (g) how many incidents of self harm have been recorded, (h) how many incidents of concerted indiscipline have been recorded, (i) how many assaults on staff have been recorded and (j) what is the total amount of payment reductions for contract failures; and if he will make a statement. [135581] Mr. Boateng: Each secure training centre (STC) is subject to comprehensive inspection by the Social Services Inspectorate (SSI), who may be accompanied by Her Majesty's Chief Inspector of Prisons and by one or more of Her Majesty's Inspectors of Schools (OFSTED), at least once every three years. There are also annual inspections to check that the secure training centre is meeting core requirements. Other inspections, with or without notice, can take place at any time of the year; and each centre is subject to a pre-opening inspection. Annual and triennial inspection reports are placed in the House of Commons Library as they become available. The Inspectors' reports show that, after significant initial operational difficulties at the first STC, Medway, both it and the newer centres at Rainsbrook and Hassockfield have progressively developed to provide a good standard, comprehensive regime for difficult young juveniles. (a) The Secure Training Order (STO) was available from March 1998 to March 2000. 379 children received STOs of whom 319 went to STCs, and the others to local authority secure units. 20 per cent. of those who went to Medway, 8 per cent. of those who went to Rainsbrook and 4 per cent. of those who went to Hassockfield were returned to custody for breach of STO supervision. The STO was replaced by the Detention and Training Order (DTO) in April 2000. Information on DTO breaches is not held centrally. (b) and (c) This information is not collected routinely but research is under way. The report of the Policy Research Bureau into the first year at Medway "Evaluation of Medway Secure Training Centre", released on 4 October 2000, recorded a 67 per cent. re-offending rate for those at that establishment. Wider research commissioned by the Youth Justice Board into DTO re-offending is being carried out by the National Association for the Care and Resettlement of Offenders (NACRO), the Centre for Crimes and Justice Studies and the Policy Research Bureau. Results are expected in March 2002. (d) Home Office monitors based at each STC ensure that the terms of the contracts including on education and training are met on a daily basis. The SSI's annual and triennial inspection reports are in the Library. (e) This information is not available. The contracts specify minimum qualifications, skills and experience requirements. All staff receive Home Office-approved training in understanding and dealing with disturbed adolescents. Jackie Ballard: To ask the Secretary of State for the Home Department how many incidents of self-harm were recorded by people held in (a) court cells and (b) on prison escort, in each of the last five years. [106113] Mr. Boateng: Information systems used by the Prison Service do not allow a differentiation between incidents of self-harm committed by people in court cells and on prison escort. The information available on self-harm incidents of people while on prison escort undertaken by private sector companies acting under contract to the Prison Service is the total number reported to the incident management support unit (IMSU) in Prison Service headquarters. These figures are given in the table. Care should be taken when interpreting the data because, although the majority of incidents of self-harm occur while in court, some incidents do take place in cellular vehicles. In addition, it should be noted that responsibility for the escorting of prisoners to and from court was progressively contracted out in England and Wales between April 1993 and August 1997. Consequently, the figures for 1995-96 and 1996-97 are low because incidents involving prisoners under escort by prison staff would have been recorded against the establishment involved, while those relating to prisoners in police custody would have been collected locally. Mr. Simon Hughes: To ask the Secretary of State for the Home Department what was the total number of (a) incidents of concerted indiscipline, (b) incidents of self harm and (c) suicides in each Prison Service establishment and in total in (i) 1999 to date, (ii) 1998, (iii) 1997 and (iv) 1992; and if he will make a statement. [102658] Mr. Boateng: The information requested is given in tables which have been placed in the Library. The Government are committed to doing everything possible to try to reduce the number of self-inflicted deaths and incidents of self-harm in prisons. The Prison Service is developing a whole range of additional measures to address this issue and to provide further help and protection to those considered to be at risk of self-harm; this work is being informed by the recommendations of Her Majesty's Chief Inspector of Prisons' Thematic Review published in May of this year. The Prison Service is also increasing the involvement of external interest groups through a regular working group and in projects and research into suicides. There has been for some time a close working relationship with the Samaritans who have a great deal of expertise in this area. Listener schemes, whereby prisoners offer support to fellow prisoners in crisis, are now established in over 100 prisons in England and Wales, the majority of which are supported by the Samaritans. In addition, instructions and guidance to staff on caring for the suicidal are being re-examined and drawn together in a new Prison Service Order (PSO) which it is hoped will be ready for issue in April 2000. This will highlight good practice and strengthen strategies to provide care for those prisoners at risk. There has been a substantial decrease in the number of concerted acts of indiscipline since 1992 (other than a brief rise in 1998). The Prison Service routinely monitors trends to detect any increase in the seriousness and frequency of acts of indiscipline. In the meantime, it continues to develop control and restraint techniques to ensure that staff can respond effectively to these incidents when they occur.
