Miss Melanie Johnson :[holding answer 13 September 2004]: Minor procedures to the feet are generally performed on an
out-patient basis, that is, without admitting the patient. There is currently no mandated collection of data on out-patient
procedures, regardless of which type of professional is responsible for the work. Any data collected locally are outside of
departmental requirements and the decision to collect and monitor such information rests with the front line organisations.

Dr. Richard Taylor: To ask the Secretary of State for Health what proportion of NHS hospitals undertaking joint replacement surgery
have a consultant haematologist or orthopaedic surgeon with special training or interest in venous thrombosis. [189454]
Mr. Hutton: The information requested is not collected centrally.
All hospitals undertaking joint replacement or any other form of surgery are expected to have the appropriate clinical skill mix to
ensure that procedures are performed safely and to a high standard.
The number of consultant haematologists and trauma and orthopaedic surgeons in England has increased by 144 (29.5 per cent.)
and 462 (43.8 per cent.) respectively between September 1997 and June 2004. Individual doctors' special interests are not
collected by the Department.

Notes: Diagnosis (Primary Diagnosis) The primary diagnosis is the first of up to 14 (seven prior to 2002-03) diagnosis fields in the
HES data set and provides the main reason why the patient was in hospital. Main operation The main operation is the first recorded
operation in the HES data set and is usually the most resource-intensive procedure performed during the episode. It is appropriate
to use main operation when looking at admission details, for example, time waited, but the figures for "all operations count of
episodes" give a more complete count of episodes with an operation. Grossing Figures are grossed for both coverage and
missing/invalid clinical data, except for 2002-03, which are not yet adjusted for shortfalls. Time Waited Time waited statistics
from HES are not the same as the published waiting list statistics. HES provides counts and time waited for all patients admitted to
hospital within a given period whereas the published waiting list statistics count those waiting for treatment on a specific date
and how long they have been on the waiting list. Also, HES calculates the time waited as the difference between the admission and
decision to admit dates. Unlike published waiting list statistics, this is not adjusted for self-deferrals or periods of

Miss Melanie Johnson: Information on non- responders is shown in the table. The overall response rate for the survey was good and we
understand the main reason for not completing the questionnaire was pressure of business. Non-responders will not be followed up as
the aim of the survey was to produce a snapshot of the situation in 2003. The survey results were used in the National Audit Office
report and there are no plans for a separate publication.

Hywel Williams: To ask the Secretary of State for Health what plans he has to extend patient choice in respect of orthopaedic
surgery. [182200]
Mr. Hutton: Patients waiting more than six months for orthopaedic surgery are already being offered the choice of faster treatment
in an alternative hospital.
By August this choice will be available to patients across the national health service in England.
By December 2005 patients will have the choice of four to five hospitals at the time their general practitioner refers them to
hospital.

(5) how many UK patients have been referred through the NHS to the Ex-Centre in Stockholm for multiple limb disability. [179096]
Dr. Ladyman: Figures on how many individuals in England have congenital multiple limb disability are not collected centrally,
although the Department is aware that there are 455 individuals currently being supported in the United Kingdom by the Thalidomide
Trust. Also, the Department recorded 18,910 finished admission episodes in national health service hospitals in England in 2002-03
relating to congenital malformations and deformations of the musculoskeletal system. A finished admission episode is the first
period of in-patient care under one consultant within one healthcare provider. Admissions do not represent the number of
in-patients, as a person may have more than one admission within the year.
It is for primary care trusts (PCTs), in partnership with strategic health authorities and other local stakeholders, to determine
how best to use their funds to meet national and local priorities for improving health, tackling health inequalities and modernising
services. This process provides the means for addressing local needs within the health community including the provision of
prosthetic limbs. Prosthetic services are delivered to patients through 33 disablement service centres in England. Patients normally
attend their local centre, but can attend another if they wish. If an overseas option is considered, it would be for the local PCT
commissioning team to agree that. Figures on how many UK patients have been referred through the NHS to the Ex-Centre in Stockholm
for multiple limb disability are not collected centrally. Prosthetic limbs are provided free of charge to single or multiple
prosthetic limb users by the NHS. The NHS has available to it, via contracts negotiated by the NHS Purchasing and Supply Agency,
virtually everything that is available on the international market. Furthermore, local healthcare professionals have the ability to
obtain products from outside of the current national agreement if it is deemed appropriate for the specific needs of particular
patients.

Mr. Burstow: To ask the Secretary of State for Health what assessment he has made of the success rates of orthopaedic procedures
undertaken by overseas surgeons on short-term contracts. [174474]
Mr. Hutton: All contracts for orthopaedic procedures, including those involving overseas surgeons, are subject to review including
audit of clinical outcomes by the employing trusts. All contracts are subject to local clinical governance policies which will
include the audit of surgical procedures. Information on clinical outcome will be held locally.

Andy King: To ask the Secretary of State for Health how many general practitioners hold a specialist interest in (a) rheumatological
conditions, (b) musculoskeletal conditions and (c) orthopaedics; and what steps are being taken to increase these numbers.

Mr. Hutton: There are now some 1,300 general practitioners with a special interest (GPwSIs). However, information on the number of
general practitioners with a special interest in particular specialties is not collected centrally.
In line with "Shifting the Balance of Power", the expansion of the range of services provided in primary care, though the
development of new staff roles, such as GPwSIs, should be locally driven. Primary care trusts are therefore responsible for
developing GpwSIs' services in accordance with their local health economy need to improve access for patients.
