Methadone remains the main treatment of the national health service for the management of opiate dependence. The main alternative drug for management of opiate dependence is buprenorphine¹, used increasingly in the NHS since the Department amended regulations in April 2001 to enable practitioners to prescribe the drug for opiate dependence safely through instalment dispensing.Patients may also be prescribed pharmaceutical diamorphine, and in a small number of cases alternative opiates such as clonidine and lofexidine to substitute for illicit opiate use, with decisions based on clinical judgement for each particular case.As part of the Government's ongoing commitment to ensuring drug treatment is delivered in a way that maximises the potential for a successful outcome, the National Institute of Health and Clinical Excellence is currently conducting a review of the effectiveness of methadone, buprenorphine and naltrexone in the treatment of drug misusers, due for publication in March 2007.Although outcomes generally are better for those who remain on opiate substitution treatment, there has been an increase in focus on commissioning effective support for patients who are ready to benefit from abstinence based approaches to optimise such treatment pathways. Rehabilitation services or aftercare support are available for those opiate dependent drug users who can successfully become drug-free and who no longer require treatments such as methadone.¹ Marketing Authorisation, Subutex

Records taken from the National Treatment Agency's national drug treatment monitoring system give data on the number of people receiving all opiate substitutes, like buprenorphine and naltrexone, and not just methadone, as part of a drug treatment programme. These figures are available for the last three years only:

Patients are told that methadone and other prescribed drugs must be kept out of reach of children, and it is required that all medicines be provided in child resistant containers.The Department published ““Drug Misuse and Dependence—Guidelines on Clinical Management (1999)””, which gives advice and assists general practitioners (GPs) and specialist drug treatment workers in giving appropriate substitute prescribing advice to patients.The guidelines¹ advise that methadone dose consumption should usually be supervised by either the patient's GP, a nurse or community pharmacist for the first three months of prescribing. In the rare event that such initial doses are to be taken unsupervised, the dose should be set to minimise the risk of unsolicited use and methadone diversion.Pharmacists are provided with training packs provided by the centre for pharmacy post-graduate education, ““Opiate treatment: supporting pharmacists for improved patient care”” which pharmacists are recommended to complete if taking part in the enhanced service to provide supervised consumption. This specifically covers the risks to children and opiate naive individuals of overdose.Best practice guidance² to pharmaceutical services for drug users was published in February 2006 to help ensure the provision of verbal and written information on the safe storage of controlled drugs. Additionally, guidance for administration of medicine within pharmacies, which includes methadone, ““The NHS community pharmacy contractual framework enhance service—supervised administration (consumption of prescribed medicines)””, was published in September 2005."¹ Drug Misuse and Dependence—Guidelines on Clinical Management, 1999.""² Best practice guidance for commissioners and providers of pharmaceutical services for drug users, February 2006 (Royal Pharmaceutical Society of Great Britain, national treatment agency for substance misuse, pharmaceutical services negotiating committee, PharMag, NHS)."

(35477)""Figures readily available from death certification relate to deaths due to drug poisoning, not the total number that were attributable to drug use. These figures are produced from a special database which contains deaths where the underlying cause was drug poisoning. The most recent year for which figures are available is 2003. The table below shows the number of deaths due to drug poisoning where methadone, heroin/morphine or any drug controlled under the Misuse of Drugs Act were mentioned on the death certificate for the five years 1999 to 2003."

(18008)""Available figures relate to the number of deaths from drug-related poisoning where methadone was mentioned on the death certificate. The most recently available information is for deaths in 2003. It is not possible to identify from death certificates which substance was the primary cause when more than one was involved. Figures related to the number of deaths where methadone was mentioned on the death certificate, either alone or with other substances, are shown in the table below. This table is taken from information published in the annual report: ““Deaths related to drug poisoning: England and Wales, 1999–2003””, which was published in Health Statistics Quarterly 25 in February 2005."

This information is not collected in the format requested. However, figures for London primary care trusts (PCTs) from 2001 to 2004, which are the latest available, are shown in the tables. These figures are not comparable as there have been changes to PCT configurations during this period. There were also small amounts of prescribing which could not be allocated to a specific PCT.Methadone can be prescribed for suppressing a cough, usually in terminal disease, for analgesia and for treating substance dependence. Figures for each of these are also shown in the tables.

There have been no formal assessments made on the level of abuse of methadone. However, the Department recognises the key role that effective substitute prescribing has to play in meeting our overall aim of offering high quality treatment to drug misusers. It is because of this commitment that the Department published Drug Misuse and Dependence—Guidelines on Clinical Management (1999), which is widely regarded as being extremely successful in assisting prescribers in offering appropriate treatment to their patients. Part of this also tackles the issue of diversion of methadone through advice on supervised consumption.Building on this, the National Treatment Agency (NTA), which has a remit to improve the availability and quality of drug treatment, has just completed an audit of prescribing services in England which will be available shortly. The NTA will also be undertaking series of reviews of community prescribing services with the Healthcare Commission, later in 2005. The NTA has performance management structures in place to effectively tackle those drug action teams assessed as underperforming, as far as provision of drug treatment is concerned.One of the key measures used to assess the quality of substitute prescribing, including levels of diversion, is the number of drug related deaths associated with its use. In 2003, the latest year for which figures are available, 167 deaths associated with the use of methadone were reported. This compares with 286 in 1999, a reduction of 42 per cent.As part of our ongoing commitment to continue to develop improved practise in substitute prescribing, the National Institute for Health and Clinical Excellence will, within a comprehensive package of work that it will be undertaking on drug treatment, do a specific appraisal on the clinical effectiveness of methadone in the treatment of drug misuse.

[holding answer 27 June 2005]: The Department and the National Treatment Agency, as part of their commitment to appropriate substitute prescribing, have produced a number of pieces of work in support of this objective. These are:"““Clinical Guidelines on the Management of Drug Dependence”” (Department of Health, 1999), which is based on an expert review of the evidence. This is a key document underpinning standards of treatment interventions, including substitute prescribing, expected of professionals.""““Research Into Practice: Prescribing Services for Drug Misuse”” (National Treatment Agency, January 2003). This document provided recommendations in relation to recommended guiding principles of an effective prescribing regime and the recommended components of an effective prescribing service.""A Chief Medical Officer letter (January 2001) relating to a change in national health service regulations that allowed for the instalment dispensing of buprenorphine. This regulation change was seen as being effective in increasing prescribers' willingness to prescribe buprenorphine as the risk of overdose and diversion was reduced substantially."As part of our continuing commitment to develop the evidence base in this area as part of our objective to increase the effectiveness of treatment, the National Institute for Clinical Excellence, as part of its 10th wave, has agreed to do an appraisal of methadone and buprenorphine in the treatment of drug addiction. This important work, which will be completed by the end of 2006, is likely to be influential in the development of appropriate and effective substitute prescribing in future years.

The most recently available information for all the countries of the United Kingdom is for deaths in 2003. It is not possible to identify from death certificates which substance was the primary cause when more than one was involved.
