Tables showing which hospital trusts in 2008-09 and 2007-08 did not meet the national standard for accident and emergency four-hour maximum waiting times have been placed in the Library.
The 18 weeks maximum referral to treatment standards came into effect from 1 January 2009. Therefore, no acute hospital trust failed to achieve the standards prior to January 2009. Performance information on acute trusts that failed to achieve one or both of the admitted or non-admitted 18 weeks standards in January, February or March 2009 is shown in the document Trusts Failing to Meet 18 Weeks Standard in 2008-09 which has also been placed in the Library.
In November 2004, the Government set the National Health Service (NHS) a national target of reducing Methicillin-resistant Staphylococcus aureus (MRSA) bloodstream infections by 50 per cent, over the 2003-04 quarterly average, by the end of March 2008. The NHS met this target on time and since then has continued to reduce levels of MRSA bloodstream infections. The latest quarterly figures (Q4, 2008-09) show that there has been a 64 per cent reduction in MRSA bloodstreams infections compared with the 2003-04 quarterly average.
The 50 per cent target was set nationally, not at trust level, as the number of MRSA bloodstream infections in individual hospitals can be very small, even zero. In such cases, it would not be realistic to expect trusts to achieve a 50 per cent reduction.
The 2009-10 operating framework recognised that some variation in performance on MRSA bloodstream infections exists across the NHS, and required those trusts with continuing high levels of preventable MRSA bloodstream infections to address this. The department’s targeted support team continue to work with these trusts as required to achieve this.

My Lords, I thank the noble Lord for that reply, although there is more than a whiff of hypocrisy about it. After all, I and many others on all sides of the House have argued that it should not be a matter of price. We have urged the British consumer to buy British bacon because of the higher welfare standards that are applied in this country. Will the noble Lord also take into account the presence in Dutch bacon of a deadly form of MRSA, ST398, which can cause skin infection, heart trouble and pneumonia? Is he not putting people in this country at risk, particularly as the strain has passed from animals to humans? Indeed, when Dutch farmers go into hospital, they go into isolation. Why is he putting the British consumer and those who buy bacon in this House at risk in this way?

My Lords, the noble Lord’s first comment was to accuse me of hypocrisy, which I find slightly strange. The question is one of price. It is our job to offer value for money to our customers in the River Restaurant. The market is price-sensitive and we charge considerably less than the House of Commons, which serves British bacon. Ours is about half the price. Bearing in mind that the staff are some of the lower paid in the Palace of Westminster, I think that that is right. It is fine for people such as the noble Lord and, indeed, me to buy British bacon for ourselves, but when we are dealing with other people we have to offer value for money. As far as MRSA is concerned, I read the article in, I think, the Daily Express a couple of weeks ago. I do not think that it has been followed up by anybody else, so I do not know whether it is true or not. If it were true, it would be a matter for the Ministry of Agriculture, Fisheries and Food and not for us to deal with.

As soon as a patient is identified as an MRSA (methicillin-resistant Staphylococcus aureus) carrier, a decolonisation regimen should be started. This is normally the use of an antibacterial shampoo and body wash daily, and the application of an antibacterial nasal cream three times a day for five days supplied by the national health service.It is for clinicians to decide on the most appropriate products for decolonisation and we do not collect information on the products used. However, the Department will be issuing guidance on products for decontamination later this year.The current policy is to screen all elective patients, except for those in certain groups for whom the risk of MRSA infection is very low. We estimated that this would require around 6.3 million tests in 2009-10. Elective patients found to be positive for MRSA will receive decolonisation treatment. The total cost of screening those patients, and decolonising those with a positive screen, is estimated at £44 million, an average of £7.02 per patient.

The data used to calculate the 65 per cent. reduction in methicillin resistant Staphylococcus aureus (MRSA) on the 2003-04 baseline were the October to December2008 national quarterly data published by the Health Protection Agency (HPA).These data show there were 676 reports in October to December 2008 and 7,700 reports in the baseline year 2003-04. The National Health Service has substantially reduced the number of meticillin-resistant staphylococcus aureus (MRSA) and Clostridium difficile cases.
Latest data published by the Health Protection Agency for October to December 2008 show a reduction in MRSA bloodstream infections of 38 per cent from the corresponding quarter in 2007 and down 65 per cent from the 2003-04 quarterly average, (the baseline from which our target to halve the number of MRSA bloodstream infections is measured). The National Health Service delivered this target of halving MRSA bloodstream infections by the end of 2007-08. We are now committed to sustaining the reductions achieved so far and continuing to drive down infections still further.
There has also been a significant reduction in the number of Clostridium difficile cases, with the latest data for July to September 2008 (England) showing a 33 per cent reduction in infections in patients aged two and over compared with the same quarter in 2007.
The department's strategy for reducing healthcare-associated infections in the NHS is set out in Clean, Safe Care, which has already been placed in the Library. This outlines the comprehensive range of measures being employed, backed by £270 million additional investment per year by 2010-11 to tackle healthcare-associated infections.

Incidents of methicillin-resistant Staphylococcus aureus in my three local hospitals have drastically reduced in the last few months, but will the Secretary of State look at the methodology currently used for determining incidents of MRSA in hospitals? In Barnsley, we had an incident where a patient had an infection after his pacemaker had been fitted in an independent treatment centre. Because it took some time to discover how he had been infected in the hospital, the infection was counted as three separate incidents of MRSA for that same patient. That seems a bit unfair to me, so will my right hon. Friend look into that?

My hon. Friend is right to point to the 65 per cent. reduction in MRSA against the baseline of 2003-04, and there has been a 47 per cent. reduction in cases of clostridium difficile compared with the same quarter last year. He raises an important point, and one case of MRSA, clostridium difficile or any hospital-acquired infection is one case too many. We must consider sensible points such as his to see whether we can eradicate another source of health care-acquired infection.

The Department was informed by Novartis on 23 February 2009 that staphylococcus aureus bacterial contamination had been found in samples of the solvent used in meningitis C vaccine. The material in question had not been distributed in the United Kingdom but came from the same manufactured batches that had been distributed in the UK. The Department reminded Novartis of its statutory responsibility to inform the Medicines and Healthcare products Regulatory Agency (MHRA). Novartis contacted the Defective Medicines Report Centre (DMRC) at MHRA on 24 February by telephone initially, then followed up with an e-mail.The MHRA issued a class 2 drug alert on 25 February 2009. The drug alert informed recipients of the issue and identified potentially affected batches.On the evening of 25 February 2009 a chief medical officer (CMO) message was sent via the central alert system (formerly known as the public health link) as a class 1, immediate action alert, for immediate cascade (within six hours) to relevant health professionals, including general practitioners (GPs) and practice nurses, accident and emergency departments and community pharmacists. At the same time a pager message was also sent that evening to all recipients of the CMO message, instructing them to immediately cascade this information to all relevant health professionals. Local immunisation co-ordinators are direct recipients of CMO messages via the central alert system, and would have received the electronic alert that night.The Department's immunisation team separately contacted all primary care trusts immunisation co-ordinators on the evening of 25 February and informed them of the GP surgeries in their areas that had received the batches to be withdrawn.The MHRA has arranged for additional sterility tests to be carried out on the affected solvent batch in the UK. A total of 120 solvent samples will be tested. The MHRA is awaiting delivery of test samples. Sterility testing takes at least 14 days, so the date on which the tests are completed will depend on when the samples are put on test. The Department was informed by Novartis on 23 February 2009 that staphylococcus aureus bacterial contamination had been found in samples of the solvent used in meningitis C vaccine. The material in question had not been distributed in the United Kingdom but came from the same manufactured batches that had been distributed in the UK. The Department reminded Novartis of its statutory responsibility to inform the Medicines and Healthcare products Regulatory Agency (MHRA). Novartis contacted the Defective Medicines Report Centre (DMRC) at MHRA on 24 February by telephone initially, then followed up with an e-mail.The MHRA issued a class 2 drug alert on 25 February 2009. The drug alert informed recipients of the issue and identified potentially affected batches.On the evening of 25 February 2009 a chief medical officer (CMO) message was sent via the central alert system (formerly known as the public health link) as a class 1, immediate action alert, for immediate cascade (within six hours) to relevant health professionals, including general practitioners (GPs) and practice nurses, accident and emergency departments and community pharmacists. At the same time a pager message was also sent that evening to all recipients of the CMO message, instructing them to immediately cascade this information to all relevant health professionals. Local immunisation co-ordinators are direct recipients of CMO messages via the central alert system, and would have received the electronic alert that night.The Department's immunisation team separately contacted all primary care trusts immunisation co-ordinators on the evening of 25 February and informed them of the GP surgeries in their areas that had received the batches to be withdrawn.The MHRA has arranged for additional sterility tests to be carried out on the affected solvent batch in the UK. A total of 120 solvent samples will be tested. The MHRA is awaiting delivery of test samples. Sterility testing takes at least 14 days, so the date on which the tests are completed will depend on when the samples are put on test.
