"Clinical Practice Guidance for the Assessment of Young Women aged 20-24 with Abnormal Vaginal Bleeding" was issued through the Department of Health's GP and Practice Team Bulletin and Chief Nursing Officer's Bulletin in March 2010. The guidance, developed by the independent Advisory Committee on Cervical Screening (ACCS), re-emphasises the importance of pelvic examinations in young women with abnormal vaginal bleeding and provides primary care practitioners with an easy to follow algorithm to help in their management of these young women. A copy of the guidance has been placed in the Library.Regarding research on the detection and diagnosis of cervical cancer in general and among those aged between 20 and 24 years in the last 12 months, NHS Cancer Screening Programmes provided funding of £52,000 in 2009-10 to Cancer Research UK on the ongoing audit of invasive cervical cancers. This included funding for specially commissioned work on younger women to inform the ACCS review of cervical screening in May 2009.In addition, in autumn 2009 the Department commissioned the "What Women Know" survey to assess the current levels of knowledge about cervical cancer among women in England. The survey was carried out as part of an initiative in collaboration with the Eve Appeal and University College London's Health Behaviour Research Centre. It covered awareness of the symptoms and risk factors for cervical cancer, knowledge of the cervical screening and human papillomavirus vaccination programmes, anticipated barriers to seeking medical help with a possible cervical cancer symptom, and beliefs about screening and other protective behaviours.The Department has also provided £4,000 to Jo's Trust, a charity that raises awareness of cervical cancer, to undertake a pilot to assess the potential for using television advertisements in general practitioner surgeries to improve uptake of cervical screening invitations.

The NHS Health Check programme is a universal and systematic programme for everyone between the ages of 40 and 74 that will assess individuals' risk of heart disease, stroke, kidney disease and diabetes and will support people to reduce or manage that risk through individually tailored advice. The programme was developed on the basis of advice from the National Screening Committee, is based on guidance produced by the National Institute of Health and Clinical Excellence (NICE) and is both cost effective and clinically effective.The risk assessment stage of the NHS Health Check uses a risk engine to calculate a person's 10-year risk of cardiovascular disease. Evidence on cardiovascular risk estimation has developed considerably, with a number of risk engines available, including Framingham and QRISK. These have been calibrated to reflect risk factors within specific populations, such as ethnicity, and a number of scientific reviews have been published on their validity. The Department takes its advice from NICE on matters such as which risk engines are most appropriate for the national health service to use. NICE has recently revised its guidance to allow the NHS to decide locally which risk engine best predicts risk for their population mix.Of equal importance however is that people are given individually tailored advice and the necessary follow up which is suitable for their particular circumstances to help them manage or reduce their risk. The advice, interventions and follow up that are offered will involve professional judgment and will help ensure that everyone's individual needs are met.

The National Health Service Health Check programme (formerly vascular checks) is a universal and systematic programme for everyone between the ages of 40 and 74 that will assess an individual's risk of heart disease, stroke, kidney disease and diabetes and will support people to reduce or manage that risk through individually tailored advice. The programme was developed on the basis of advice from the National Screening Committee. It advised that, on current evidence, there was not a case for whole population screening for diabetes. However, there was a good case for targeted screening for diabetes in the wider context of cardiovascular risk assessment. The department undertook modelling on a vascular risk assessment and management programme, which included diabetes to establish whether such an approach would be cost and clinically effective as well as identifying the optimal starting age. Both the technical consultation on the modelling and the impact assessment are available in the House of Lords Library. Three different starting ages for the programme (40, 45 and 50) were modelled. The age range of 40 to 74 for everyone was found to be both clinically and cost effective. In taking the decision about the age range with which to launch the programme, the department took further advice from the National Screening Committee and leading United Kingdom diabetologists. Their view was that, given the current state of knowledge, age 40 represented a good starting point for combined diabetes, cardio-vascular and kidney disease risk assessment. The content and age parameters of the programme will be kept under review. We understand that some primary care trusts maybe inviting people into the programme who are under 40 years of age, for example, in areas that have significant South Asian communities who are generally at higher risk of vascular disease including diabetes. However, this scenario was not tested through the modelling, and so we have no evidence of its clinical or cost effectiveness.

The Department has made no specific assessment of the effectiveness of the national health service in diagnosing tuberculosis. The Care Quality Commission, as part of its Annual Health Check of NHS organisations, monitors the implementation of National Institute for Health and Clinical Excellence (NICE) guidelines and assesses how the NHS implements delivery of services, including TB services. The organisation and running of local services are matters for local NHS management. Primary care trusts are responsible for procuring TB services to fit their local demography and incidence of TB in their area, and ensuring that those standards are monitored, met and reviewed as part of the commissioning process.The Department published a toolkit in June 2007 for the NHS recommending that the NICE clinical guidelines (published 2006) on the diagnosis and management of tuberculosis should be followed by all NHS service providers.The diagnosis of TB is made from a combination of context, symptoms, clinical signs and investigation. NICE recommends that clinical investigation for pulmonary TB (the only infectious form) comprise a chest X-ray, followed by multiple sputum samples for TB microscopy and culture. These represent the current 'gold standard' diagnostic tools, and NICE advises that if the clinical signs and symptoms are consistent with pulmonary TB then treatment should be started without waiting for further tests.NICE identified areas for future research, one of which concerned Interferon Gamma Release Assay (IGRA) tests, to assess whether interferon-gamma tests are acceptable to patients and more effective than tuberculin skin tests for predicting subsequent development of active TB, or diagnosing or ruling out current active TB. Research proposals have been set in motion for these, but results of these studies are likely to be some years away.

The NHS Abdominal Aortic Aneurysm screening programme has cost £5,335,809.50 up to the end of March 2010. This is made up of start up and full year costs for six early implementation sites, start up costs for four phase one sites, screening equipment for 10 sites, training of technicians and information leaflets.It is anticipated that there will be a maximum of eight phases of roll out, however the exact number is unknown as this depends on the number of applications received and geographical fit of each local screening programme.The following national health service trusts have been successful with their applications to begin ultrasound screening for aortic aneurysms:
