The National Health Service is making significant process toward a zero tolerance approach to all avoidable methicillin-resistant staphlylococcus aureus (MRSA) bloodstream infections. Over the past 12 months, reported MRSA bloodstream infections have fallen by 29 per cent.

The Government continue to encourage the national health service to operate a zero tolerance approach to all avoidable methicillin- resistant Staphylococcus aureus (MRSA) bloodstream infections.The NHS Operating Framework 2012-13 (published 24 November 2011) includes a MRSA objective which will require those organisations who have the highest rates of MRSA bloodstream infections to make the largest reductions.In this way we will continue to build on the significant progress already made by the NHS and move the performance of all organisations towards the level of the best and, consequently reduce infection rates further.

We are advised by the Food Standards Agency, which has responsibility for food hygiene matters, that there are no plans to introduce new legislative proposals to reduce the incidence of methicillin-resistant Staphylococcus aureus (MRSA) in milk.Current evidence suggests that there is a negligible food safety risk from MRSA bacteria in milk. The vast majority of milk sold for human consumption is pasteurised and this process is effective in destroying bacteria present in the milk, including any MRSA.

[holding answer 15 June 2011]: The former Secretary of State for Environment, Food and Rural Affairs, the right hon. Member for Blackburn (Mr Straw) and Ministers were first informed of the novel MRSA strain that had been identified by the university of Cambridge on 24 September 2009. On 1 June 2011, the Secretary of State for Environment, Food and Rural Affairs, my right hon. Friend the Member for Meriden (Mrs Spelman), was notified of the impending publication of the university of Cambridge's paper on this novel MRSA strain.As a result of the university of Cambridge notifying the Department of the identification of this novel strain in 2009, DEFRA, including its Veterinary Medicines Directorate (VMD) and Veterinary Laboratories Agency (VLA), alerted colleagues from the Department of Health, the Health Protection Agency and the Food Standards Agency about the initial findings. As a result of the consequent discussions, DEFRA commissioned the university to undertake some further work to enhance understanding of the origin and the significance of this new finding. Included within this additional work was the investigation of some cattle Staphylococcus aureus isolates that the VLA had retained as an archive after a research project into mastitis (inflammation of the udder) in cattle. None of these isolates were identified as MRSA by the VLA using normal testing protocols, although a few showed some resistance to the antibiotics commonly used to identify MRSA. These isolates identified by VLA as having a degree of resistance underwent further examination during this additional research and some were shown to be similar to the strain identified by Cambridge.Now that this research has been completed, DEFRA will continue to collaborate with public health bodies that are all represented at the DEFRA Antimicrobial Resistance Coordination (DARC) group. A sub-group specifically on MRSA will also continue to monitor all new developments relating to all types of MRSA and will advise on potential policy options accordingly. Further research projects will be commissioned on antimicrobial resistance. Their scope will continue to be prioritised, and this will be informed by the DARC group, which assesses risk to help target funding to where it is most needed.The Animal Health and Veterinary Laboratories Agency will continue to test clinical samples from cows with mastitis (and other diagnostic submissions from cows and other farmed species) and so monitor such material for the presence of Staphylococcus aureus and whether such isolates demonstrate resistance to antibiotics. The VMD is responsible for authorising all veterinary antimicrobial products and monitors the overall volume of sales and any suspected adverse reactions (including being notified about suspected failures in effectiveness, which may be an indication of the development of antimicrobial resistance).

Many laboratory methods are available to national health service hospitals to detect Methicillin-resistant Staphylococcus aureus (MRSA) and these meet the European Commission standards for safety, quality and performance. NHS trust laboratories use simple tests to detect MRSA. These tests have been available for many years and are capable of detecting the emergence of a new MRSA on this basis.

Any diagnostic test of this type should be CE marked by the manufacturer according to the safety, quality and performance requirements of the In Vitro Diagnostic Devices Directive (transposed in to United Kingdom law by the Medical Devices Regulations 2002) before they are placed on the UK market. Under the directive, national regulatory authorities, who in the UK are the Medicines and Healthcare products Regulatory Agency, are not directly involved in the pre-market conformity assessment process for such tests and therefore hold no information on the product's current approval status.

A variety of methods are available to national health service trusts to screen against Methicillin-resistant Staphylococcus aureus. Individual trusts make their own decisions on which tests or combinations of tests to use depending on their clinical case mix and workload. The local clinical need will determine test selection. Rapid and more sensitive methods have been available to the NHS for a number of years and trusts make their own decisions on matching the technologies to their service needs.In 2004, as part of the Department's commitment to ensuring the NHS has access to effective infection prevention and control technologies, it has asked the Health Protection Agency to convene the Rapid Review Panel (RRP). This provides a prompt assessment of new and novel equipment, materials and other products or protocols that may be of value to the NHS and social care in the prevention and control of hospital associated infections.

Acolyte, with external venture capital, looked to exploit this technology for the detection of the MRSA bacterium in the civilian sector via the BacLite product. The Ministry of Defence (MOD) has not made any assessment of the BacLite product with respect to its MRSA detection capability.Following the liquidation of Acolyte in October 2009, the original intellectual property rights underpinning the failed BacLite product reverted to the ownership of Dstl. As the development of a product to detect MRSA is not part of the Department's core business, we have no intention to resurrect the BacLite product for such purposes.

There are four types of suitable Meticillin resistant Staphylococcus aureus (MRSA) screening methods. These are:"1. Polymerase Chain Reaction (PCR test) of nucleic acids (rapid testing);""2. Rapid enrichment and immunomagnetic/bioluminescent detection;""3. Chromogenic Agar plating (direct culture); and""4. Broth enrichment culture followed by agar subculture."Decisions regarding what type of test to use either in isolation or combination are made locally based on clinical protocols. The Department has not conducted an assessment of the effectiveness of the different screening methods.Diagnostic MRSA screening tests for any of these types should be CE marked by the manufacturer according to the safety, quality and performance requirements of the EC in vitro diagnostic devices directive (transposed in to United Kingdom law by the Medical Devices Regulations 2002) before they are placed on the UK market.

There is currently published data available for the first 11 months of 2010, providing details of the number of methicillin-resistant Staphylococcus aureus (MRSA) bacteraemias (bloodstream infections). A full year's data for 2010 will be available when data for December 2010 are published on 2 February 2010.Published monthly data indicate that between January 2010 and November 2010 there were a total of 1,514 MRSA bacteraemias reported by acute national health service trusts in England. Of this number, 766 were apportioned to acute trusts."Notes:""1. All acute trusts in England are obliged to report all cases of MRSA bloodstream infection to the Health Protection Agency (HPA). Data on other MRSA infections, that did not enter the bloodstream, are not included as they are not part of this mandatory surveillance system.""2. Trusts report all cases for which their laboratories process the specimens. This may not always reflect where the infection was acquired.""3. MRSA bacteraemias are considered to be "trust apportioned' cases if they meet all of these three criterion:""the location where the specimen was taken is "acute trust' or is not known;""the patient is an in-patient, emergency assessment, or is not known; and""the date that the specimen was taken was on or after day three of admission to hospital (where the day of admission is day one)."

Although the national health service has made significant reductions in the number of Methicillin-resistant Staphylococcus aureus (MRSA) bacteraemias (bloodstream infections), this Government are determined to do all they can to support the health and adult social care providers to make further improvements. From the outset, through the coalition agreement, this Government made clear that they expected the NHS to adopt a zero tolerance approach to all health care associated infections (HCAIs), including MRSA.The ““NHS Operating Framework 2011-12””, published in December, requires the NHS to continue prioritising the achievement of the MRSA Objective, as well as the new Clostridium difficile (C. difficile) Objective. These Objectives, through their successful implementation, will have a significant impact on reducing variation by moving all organisations towards the performance of the best and consequently will at the same time reduce numbers at a national level.We have also, from the end of December 2010, extended the previous requirement for the NHS to screen all relevant elective admissions to also include relevant emergency admissions.To support the wide availability of data to help drive further reductions in MRSA, we have introduced weekly publications of data for both MRSA bloodstream infections and C. difficile infections at hospital-site level.We are also committed to ensuring that the NHS continue to have access to evidence-based guidance in order to reduce the number of all HCAIs, including MRSA, through the implementation of effective infection prevention and control practices. As part of this commitment, a comprehensive suite of updated guidance was published on the Department's ““Clean, Safe Care”” website on 6 January 2011.The Health and Social Care Act 2008 ““Code of Practice for health and adult social care on the Prevention and Control of Infections and related guidance””, which the Care Quality Commission use as a basis for assessing compliance with the registration requirement on cleanliness and infection control, has been a driver for improvement in the hospital setting. We extended the scope of the Code to adult social care settings from October 2010 and will include primary care in due course so that we can ensure that all settings where patients receive care and treatment operate comparable infection prevention and control practices.

This Government are determined to do all they can to support the health and adult social care providers reduce Methicillin-resistant Staphylococcus aureus (MRSA). The successful implementation of this objective will deliver both an overall reduction nationally and, importantly, will reduce variation by moving all organisations towards the performance of the best.At the same time, the revision of the Operating Framework confirmed that it expected and required the NHS to implement MRSA screening of all relevant emergency admissions by the end of this year.[n terms of using the availability of data as a driver to supporting further reductions in MRSA, we have introduced weekly data publication of both MRSA blood stream infections and Clostridium difficile infections at hospital site level.We are also committed to ensuring that the NHS continue to have access to evidence based guidance in order that they can reduce the number of all HCAIs, including MRSA, through the implementation of effective infection prevention and control practices. This guidance is available on the Department of Health's ““Clean, Safe Care”” website.The Health and Social Care Act 2008 ““Code of Practice for health and adult social care on the Prevention and Control of Infections and related guidance””, which the Care Quality Commission use as a basis for assessing compliance with the registration requirement on cleanliness and infection control has been a driver for improvement in the hospital setting. The scope of the Code has already been extended to adult social care settings and will include primary care in due course so that we can ensure that all settings where patients receive care and treatment operate comparable infection prevention and control practices.

As outlined in the ““NHS Operating Framework 2010/11””, there is a requirement to introduce Methicillin-resistant Staphylococcus aureus screening. Good progress is being made by organisations to implement screening for this cohort of patients, with some organisations already having declared full implementation of the policy and all organisations planning to implement emergency screening for relevant emergency admissions within the expected time scale. Strategic health authorities will continue to monitor delivery towards the requirement by 31 December 2010.

Methicillin-resistant Staphylococcus aureus (MRSA) bacteraemia are subject to mandatory reporting to the Health Protection Agency. The Department uses the outputs from this system to assess and monitor rates of MRSA bacteraemia at both national and local levels.
