To ask the Secretary of State for the Home Department, what assessment she has made of the potential merits of including a downloadable paper application form for the EU Settlement Scheme on the Government website.
To ask the Secretary of State for the Home Department, what assessment she has made of the potential merits of including a downloadable paper application form for the EU Settlement Scheme on the Government website.
Where a person needs to apply to the scheme using a paper application form, this can be obtained from the EU Settlement Resolution Centre. This is open seven days a week to provide assistance to applicants over the telephone and by email, including in ensuring they can easily obtain the paper application form appropriate to their circumstances where they need one.
Assistance for applicants to the EU Settlement Scheme is also available via a network of 72 organisations across the UK, for which £22 million in grant funding has been made available by the Home Office, and via the Assisted Digital service, which can provide help over the telephone or in person in completing an application online.
To ask Her Majesty's Government why the declaration form for international travel from England does not allow a British citizen to provide an overseas address.
To ask Her Majesty's Government why the declaration form for international travel from England does not allow a British citizen to provide an overseas address.
The travel declaration form is designed for the passenger to state who they are and why they are departing England to travel overseas. The details on the form including the address are not captured or recorded centrally. If a police officer has a reasonable belief that a passenger is travelling without a reasonable excuse, a separate process of issuing a Fixed Penalty Notice is commenced which includes recording the residential address of the passenger for enforcement purposes.
We will amend the way the address is requested on the form to make it easier to complete for passengers with non-UK addresses.

To ask the Secretary of State for Health and Social Care, what plans he has to change the requirement that a disabled applicant must manually sign form FP92A.
We have no plans to do so. When a disabled applicant is unable to complete or sign form FP92A, the correct procedure is for a general practitioner or other healthcare professional to complete the application on the patient’s behalf. This instruction is noted on form FP92A in the patient declaration section.

In accordance with the Abortion Act and the Abortion Regulations 1991, all abortions performed in England must be notified to the Chief Medical Officer within 14 days of the procedure. There is no requirement for the practitioner to wait until day 14 to notify the abortion.
Any complication known to the practitioner terminating the pregnancy, at the time the notification is submitted, should be recorded on the Abortion Notification (HSA4) form. All serious incidents should be reported by the provider to their commissioner, the Care Quality Commission and other relevant organisations in line with the serious incident framework published by NHS England and NHS Improvement which is available at the following link:
https://improvement.nhs.uk/resources/serious-incident-framework/
We acknowledge that there are limitations with the data provided on HSA4 forms as complications that occur after treatment may not be known to the registered medical practitioner and may not be reported. We are planning to examine, with partner organisations, how well these systems are working in relation to recording complications arising from abortions and whether improvement is required.
