The Child Death Notification Form and the Child Death Reporting Form are part of the National Child Mortality Database. These continue to capture the sex of the child for under 18-year-olds, in line with sex being a protected characteristic under the 2010 Equality Act.
The Child Death Overview Panel (CDOP) has a statutory obligation to notify the National Child Mortality Database within 48 hours of a child’s death, and they do so through A Child Death Notification Form. There is no reference to the mother’s gender in this form.
Following this, the Child Death Reporting Form and relevant supplementary reporting forms are sent out by the CDOP to every professional who has had contact with the child during life, or who has been involved in the investigation after death, to gather information when a child dies. In the Child Death Reporting Form, there is space for the professional to provide information on the gender of the mother. This is not a mandatory requirement to supply this information. This question has existed on the reporting form since at least 2010, and pre-dates the National Child Mortality Database.

The Government does not require the same forms annually, and therefore we cannot provide this information. However, we are committed to improving the services we offer to farmers. Through ongoing service design and research with farmers, we continue to optimise our farming schemes, including the administration they include, to make published content clear and by iterating form design, so they work efficiently for farmers, food security and the environment, especially for those that are too often ignored such as small, grassland, upland and tenanted farms. This is a priority for the new Government and we will announce more information in due course.
In the meantime, the Animal and Plant Health Agency’s (APHA) strategic programme is focused on enhancing and streamlining services, including customer communications, application processes, forms, and data collection. This initiative includes multiple projects aimed at understanding and alleviating the administrative load faced by farmers and other stakeholders.
As part of this programme, APHA has already conducted extensive user research, engaging customers across various sectors to identify their challenges and pain points. By listening directly to those who use its services, APHA is gaining valuable insights that allow it to evaluate and test solutions to make interactions with APHA smoother and more efficient.

Following the death of a child, it may not always be a healthcare professional making the notification and therefore these questions may be answered by a range of different professionals, for instance those in healthcare, police, or social care. The questions will be answered based on what the professional knows at the time when they are making the notification of death. Therefore, if they do not hold any information on the gender identity of the child, this question will not be answered.
Parents would not be completing these forms. The purpose of the notification form is to give the basic information on what is known about the child by the professional at the time of death.

The Ministry of Defence (MOD) has overhauled safety policy beginning in 2020 to provide clear direction that all suspected or confirmed heat illness cases must be reported, and to improve the quality of reporting safety occurrences. The MOD is now moving to a single safety occurrence reporting system known as MySafety.
Individual Defence organisations’ safety occurrence reporting procedures instructs their personnel on ‘how’ to report safety occurrences and the Commanders Guide to Joint Service Publication (JSP) 375 Chapter 41 (Heat Illness Prevention) specifies ‘what’ minimum data is required for reporting heat illness cases, for example: the time, location, Wet Bulb Globe Thermometer reading, Met Office weather forecast, type of activity being undertaken, and a description of the illness or injury.
The MOD has also developed and implemented heat illness prevention training that must be completed by all military personnel at the earliest opportunity (for example in Phase 1 training for new entrants) to reinforce the heat illness prevention policy, including reporting, that is set out in JSP 375 Chapter 41.
While work was undertaken in 2020 to move to a more common set of reporting and investigation forms, several individual Defence organisations continue to use separate reporting systems to report incidents of suspected or confirmed heat illness including exertional heat illness. The Army and UK Strat Com have already adopted MySafety as a unified reporting system. The remaining Front Line Commands are expected to follow suit by April 2025. MySafety has already seen an increase in reporting numbers and quality, but the full benefits will not be realised until Full Operating Capability is reached in October 2025.

Under the current system, the Lasting Power of Attorney (LPA) is a prescribed paper form, which to be used must be made validly and registered by Office of the Public Guardian (OPG). The form can be completed online but must then be printed, signed, and sent to OPG for processing.
LPAs are private agreements between individuals. While the OPG has a role in checking these documents meet the requirements for registration prior to use, and in investigating any alleged misuse, it cannot prevent an eligible person from making one.

DLA Child is currently undergoing transformation as part of DWP’s Service Modernisation Programme. Work is underway to explore options including how to apply, with the aim of providing a sustainable, simplified and accessible service and the ability for customers to self-serve.

The Government wants to encourage as many people as possible to complete their self assessments online. HMRC therefore asks taxpayers who do not initially opt to complete their return online to call HMRC. This approach allows HMRC to speak to taxpayers to encourage them to file online, or to find out if they need additional support and guidance to do so, before sending them a paper form if needed. The SA100 form is available for download on gov.uk.

The system to process possession claims when section 21 is abolished already exists for claims under Section 8 grounds of the Housing Act 1988 and the Civil Procedure Rules.
There will be some form changes to remove the Section 21 process. Any changes required will be made by the relevant Procedure Rule Committee once the details of the new legislation are confirmed. Guidance materials for court staff will also be updated accordingly. Judicial training is the responsibility of the Lady Chief Justice and the Judicial College and will be considered as part of wider implementation plans.

HMRC regularly encounters attempts to fraudulently attack its systems and does not routinely keep records of the value of individual instances unless a pattern emerges.
Towards the end of February, HMRC identified a small number of cases which constituted more than isolated incidents. In these cases, paper VAT 484 forms had been wrongly used in an attempt to fraudulently gain access to VAT repayments. Immediate steps have been taken to prevent this from happening, and the situation is being monitored to see if further action is necessary.
