The Government recognises that access to National Health Service funded fertility services is variable in England and is looking into achievable ambitions to improve access to fertility services. This includes understanding the offer made by fertility services across the country with a view to assessing the data collected from integrated care boards to evaluate next steps, working to ensure there is equitable access.

All deaths in England and Wales must be reviewed by a medical examiner. Medical examiners are senior doctors who have not been involved in caring for the deceased, and provide independent scrutiny comprising three elements:
- a proportionate review of medical records;
- a review of the causes of death proposed by the attending practitioner and consideration of whether the coroner needs to be notified; and
- an opportunity for bereaved people to ask whether they have questions about the cause of death or concerns about the care of the deceased person before death.
The medical examiner completes a declaration on the Medical Certificate of Cause of Death that the causes of death are true to the best of their knowledge and belief.
The Notification of Deaths Regulations 2019 set out the types of deaths that medical practitioners must notify to senior coroners, for example, if the death is unnatural, if the cause of death is unknown, or if it is due to the use of a medicinal product. Where a death has been notified to the coroner, the coroner will decide whether they have a duty to investigate the death. If they do investigate, the coroner will focus on establishing who has died, where and when they died and how they died.

Further to the answer provided on 28 September in response to question HL3452, since 1 January 2025, fulfilment of private prescriptions for puberty blockers for people under 18 years old with gender incongruence or gender dysphoria has been banned under the provisions of section 62 of the Medicines Act 1968. This includes prescriptions issued by prescribers in the European Economic Area (EEA) or Switzerland.
While United Kingdom pharmacies can otherwise dispense prescriptions issued by appropriately regulated prescribers in the EEA or Switzerland, they can refuse to dispense a prescription if they have concerns about whether it is genuine, clinically appropriate, or safe. The Government is currently considering the findings of our recent call for evidence on non-National Health Service prescribing, including on the recognition of EEA and Swiss prescriptions in the UK.
The regulatory oversight of clinics operating outside the England, and not providing regulated activities in England, is a matter for the governments in the jurisdiction in which those clinics are based.
The General Medical Council is clear that a decision about whether to enter into a shared care agreement is for the general practitioner (GP) to make. NHS England has provided specific guidance advising GPs against shared care agreements with unregulated providers in relation to hormone medication to children and young people under 18 years old as a response to gender incongruence or gender dysphoria.

Safeguarding is firmly embedded within the core duties and statutory responsibilities of all organisations that provide National Health Services. The organisations that manage adult gender dysphoria clinics will have reference to NHS England's Safeguarding Accountability and Assurance Framework, 5th edition, published 2026, that sets out the safeguarding roles and responsibilities for NHS-commissioned providers and sets out the legal framework for safeguarding adults.

All women in England and Wales should have access to safe, regulated abortions on the National Health Service under our current laws.
Safeguarding is an essential aspect of abortion care, and abortion providers are required to have effective arrangements in place to safeguard children and vulnerable adults accessing their services. Providers must ensure that all staff are trained to recognise the signs of potential abuse and coercion and know how to respond.
As part of standards set by the Care Quality Commission, abortion services must be able to prove that they have processes in place to ensure that all women and girls are seeking services voluntarily. The Department’s Required Standard Operating Procedures also require staff to be able to identify those who require more support than can be provided in the routine abortion service setting, including where there is evidence of coercion.
Section 9 of the Public Order Act 2023 introduced Safe Access Zones outside abortion clinics in England and Wales meaning that it is illegal for anyone to do anything that intentionally or recklessly influences someone’s decision to use abortion services, obstructs them, or causes harassment or distress to someone using or working at these premises. The law applies within a 150 metre radius of the abortion service provider.

There are no ‘early diagnosis centres’ for osteoporosis as such. Many patients with osteoporosis will be diagnosed in primary care. Additionally, osteoporosis assessment services can be run from a number of different specialities, for example rheumatology, endocrinology, or older people’s medicine.

There are currently no plans to make an assessment of the potential merits of a dedicated clinic for nuclear test veterans.
All veterans, including those affected by nuclear testing are able to access a range of National Health Services to meet their clinical needs. British nuclear test veterans who are concerned about their heath should discuss this with their general practitioner.
Over 99.6% of primary care networks in England have at least one general practice accredited under the veteran friendly scheme which raises awareness of the specific needs of veterans.
