We would expect healthcare professionals to follow the National Institute for Health and Care Excellence (NICE) clinical knowledge summary on osteoporosis and the prevention of fragility fractures. The clinical knowledge summary outlines the steps that healthcare professionals should take in the assessment and management of osteoporosis.
A diagnosis of osteoporosis is made following clinical assessment and in some cases by use of blood tests and DEXA scans. The National Health Service does not provide universal osteoporosis screening, instead, a targeted approach is used to identify those most likely to benefit from assessment and treatment. Tools like FRAX are recommended to calculate 10-year fracture risk and guide clinical decisions. A DEXA scan can also be used to measure bone mineral density.
There are a number of NICE recommended treatments for osteoporosis. Since 2022, two new drugs have been recommended by NICE for the treatment of osteoporosis in post-menopausal women, abaloparatide and romosozumab. These medications help to strengthen the bones and prevent bone loss, reducing the risk of fractures.

The Royal College of General Practitioners also has an e-learning module for general practitioners on the diagnosis and management of osteoporosis, developed in collaboration with the Royal Osteoporosis Society, which is designed to support the early diagnosis of osteoporosis by highlighting which groups are at higher risk of osteoporosis and fragility fractures.
In March this year, the Government announced funding for 20 new bone density, or DEXA, scanners across England, supported by £2.6 million of investment. Tens of thousands of patients will benefit from faster access to bone scans as a result, helping to ensure that people with bone conditions, such as osteoporosis, get diagnosed earlier. This builds on the first wave of 13 DEXA scanners announced last year.
Last year in 2025, over 21,000 extra DEXA scans were delivered compared with the previous year, 2024.

Ensuring patients receive their diagnostic test results, including dual-energy X-ray absorptiometry (DEXA) results quickly is a priority for the Government.
NHS England guidance, published in August 2023, sets out that imaging reports must be provided within four weeks, or 28 days, of image acquisition. All National Health Service providers and imaging networks are expected to meet this. The guidance is available at the following link:

Achieving this relies on good digital connectivity, IT infrastructure, home working solutions, and approved insourcing models established across imaging departments and networks. That is why the Government is investing in digital diagnostic transformation through NHS England’s Diagnostics Digital Capability Programme which ensures networks have a core set of digital capabilities to improve the quality, safety, and productivity of care.

There are no ‘early diagnosis centres’ for osteoporosis as such. Many patients with osteoporosis will be diagnosed in primary care. Additionally, osteoporosis assessment services can be run from a number of different specialities, for example rheumatology, endocrinology, or older people’s medicine.

We are supporting better care for patients with musculoskeletal (MSK) conditions, including osteoporosis, through the Getting It Right First Time Programme (GIRFT) for Rheumatology. The GIRFT rheumatology programme is supporting the National Health Service to deliver care more equitably across the country and improve services nationally.
Additionally, the GIRFT community MSK workstream is working to better enable integrated care systems to commission the delivery of high-quality MSK services in the community, including for patients with osteoporosis.
