Any individual, typically healthcare workers, at on-going risk of exposure to the monkeypox virus will be offered two doses of smallpox vaccine with a minimum interval of 28 days between doses. However, a second dose within 28 days is not recommended. The vaccine programme for high-risk gay, bisexual and other men who have sex with men will also offer two doses, although the offer of a first dose to as many individuals as possible will be prioritised. Data on smallpox vaccinations administered since May 2022 is continuing and will be available in due course.

On 21 June 2022, the UK Health Security Agency (UKHSA) published a targeted vaccine strategy which recommends that gay, bisexual and other men who have sex with men at higher risk of exposure to monkeypox should be offered the vaccine, which is available at the following link:

The UKHSA anticipates that the number of people in this cohort will be in the tens of thousands. Vaccines are also being offered to healthcare workers at the highest risk of exposure, including those who care for patients with suspected or confirmed monkeypox in High Consequence Infectious Disease units and sexual health centres and staff regularly undertaking environmental decontamination. While post-exposure vaccination is also recommended in specific circumstances, these individuals will be identified and contacted directly.

The UK Health Security Agency (UKHSA) is currently considering a potential vaccine strategy and the associated procurement options. Vaccines are offered to any named close contacts identified, with eligibility kept under review. Imvanex, a smallpox vaccine, has been recommended for post-exposure prophylaxis against monkeypox by an expert group. As monkeypox is caused by a virus similar to that of smallpox, this vaccine is considered effective in preventing or reducing the severity of monkeypox.
Monkeypox has low transmissibility and is spread from person to person through direct contact, with clothing or linens used by an infected person or potentially by close respiratory contact via coughing or sneezing by an infected individual.

There are no active vaccination programmes against smallpox or anthrax for any of the emergency services.
Vaccines can be made available to first responders responding to a confirmed anthrax incident if required. The Government still holds the smallpox vaccine for use against certain strains of orthopox viruses if required.

Vaccination of a cohort of front-line health workers to deal with any initial suspected or confirmed case of smallpox if one were to occur was completed in 2005. The cohort comprised a total of 516 vaccinated personnel of whom 147 were doctors, 164 were nurses, 100 were ambulance staff, 32 were scientists and 73 held other related health care occupations. All were carefully screened and monitored and none had adverse complications that required vaccinia immunoglobulin. The Department's smallpox policy is currently under review.

The Home Office has confirmed that since 2005 it has not commissioned a vaccination programme against smallpox for police emergency responders. The department’s smallpox policy is currently under review.
