I must confess to a little confusion. As regards an explanation being given and steps being taken to ensure that the same incident does not happen again, which I believe is also addressed in Amendment No. 33, I believe that on Second Reading I said rather loftily that the health service always learnt from its mistakes, that people working in the health service always did so, and that there were protocols to deal with those matters. However, I was rather put down by the Minister who said that the health service did not always investigate its mistakes. Therefore, I was persuaded by colleagues in the health service and by my noble friends in this House that the amendment should be accepted. I am glad to note that the noble Earl and his noble friends also consider that the amendment should be accepted. The Minister needs to explain why he has changed his mind on the matter since Second Reading.
