We have made no assessments of the economic effects of early diagnosis and treatment for those with myalgic encephalomyelitis.All health professionals recognise the importance of early diagnosis and treatment, indeed this is a key feature of the national service framework for long-term conditions. However, some neurological conditions, such as myalgic encephalomyelitis, can present a particular challenge for early identification as they lack clear, simple diagnostic features. Such conditions will require further investigations and clinical tests before a final diagnosis is made.

Government do not specify the content of the general practitioner (GP) training curriculum. This is the job of the Postgraduate Medical Education and Training Board (PMETB), which is the competent authority for postgraduate medical training in the United Kingdom. PMETB is an independent professional body.Regulations specify seven competencies which must be tested, and which the curriculum must therefore teach:"factual medical knowledge sufficient to enable the doctor to perform the duties of a GP; the ability to apply factual medical knowledge to the management of problems presented by patients in general practice;""effective communication, both orally and in writing;""the ability to consult satisfactorily with general practice patients;""the ability to review and critically analyze the practitioner's own working practices and to manage any necessary changes appropriately;""clinical skills; and""the ability to synthesize all of the above competencies and apply them appropriately in a general practice setting."GPs are expected therefore to learn in trailing the skills needed to deal with all their patients.

The Department submitted a proposal to the National Institute for Health and Clinical Excellence (NICE) to develop guidelines on the identification and treatment of depression in children and young people. These guidelines were published by NICE in September 2005 and include advice on detection and recognition of depression. The document, Depression in children and young people is available on NICE'S website at www.nice.org.uk.

In 2003–04, there were 12,162 finished consultant episodes in NHS hospitals in England for people with a primary diagnosis of sleep apnoea. HES data does not reflect the number of patients with a particular condition as a person may have more than one finished consultant episode (FCE) in the year. Data on the number of people with sleep apnoea is not available centrally but the British Snoring and Sleep Apnoea Association estimates that 4 per cent. of men and 2 per cent. of women are affected by the condition.Hospital episode statistics (HES) contains details of patients admitted to and treated in national health statistics hospitals in England. In 2003–04, there were 12,162 finished consultant episodes in NHS hospitals in England for people with a primary diagnosis of sleep apnoea. HES data does not reflect the number of patients with a particular condition as a person may have more than one finished consultant episode (FCE) in the year. A FCE is defined as the first period of in-patient care under one consultant within one healthcare provider.The national service framework (NSF) for long-term conditions is a 10-year programme intended to stimulate long term and sustained improvement in services principally for people with neurological conditions. It is for the NHS locally to set their own pace of change. Through the regular review and inspection processes the NHS and local authorities will need to demonstrate that they are making progress in planning and developing the level of service quality described in the NSF.Information on waiting times for inpatient and outpatient treatment in England is collected by consultant speciality rather than for specific conditions. Depending on the clinical needs of the patient, a range of consultants can undertake the treatment of sleep disorders, including neurological and respiratory specialists. As specialty level data for relevant specialties would include data for a number of conditions it is not possible to determine what proportion of suspected obstructive sleep apnoea patients wait for longer than one month to see a specialist.By the end of 2005, the maximum waiting time for first outpatient appointment with a consultant will fall to 13 weeks and the maximum waiting time for inpatient treatment will fall to six months. The child health development programme, which provides for health and development checks for young children, is informed by professional guidelines, ““principally Health for all children”” edited by Professor David Hall and Dr. David Elliman. This includes guidance on detecting abnormalities in skull development.Positional plagiocephaly is caused by allowing consistent pressure to be applied to one part of a baby's skull over a period of time. This is commonly remedied by enabling babies to experience a range of positions rather than remaining in one position. The Department's guidance to parents on the first five years of their child's life ““Birth to five””, and on reducing the risk of cot death, advocates this procedure.Funding associated with avoiding positional plagiocephaly is not separable from the overall funding of child health in the community.

Although the specific cause of sudden death by epilepsy (SUDEP) is unknown, the risks are considered greater for patients whose epilepsy is poorly controlled. Action to reduce SUDEP has therefore concentrated on improving epilepsy is poorly controlled. Action to reduce SUDEP has therefore concentrated on improving epilepsy services in general, raising awareness of SUDEP, and the introduction of new more effective drugs to control seizures.As part of the ““Action Plan on Epilepsy'' the Modernisation Agency ran ““Action on Neurology”” in 2004–05 at eight pilot sites. These included:"delivering high quality epilepsy service without the need for patients to travel far by the use of electronic and paper patient-held records.""improving communications between primary, secondary and tertiary care by developing a network of healthcare professionals to deliver a coordinated service.""developing general practitioner services for people with epilepsy and headaches and a primary care triage system to improve access and efficiency."The national service framework for long-term conditions was published in March 2005. This focuses on improving neurological services across the country for a range of conditions including epilepsy.The National Institute for Health and Clinical Excellence (NICE) has published guidance on the clinical and cost effectiveness of new epilepsy drugs in adults (March 2004) and children (April 2004).We have commissioned NICE to produce clinical guidelines for the diagnosis, management and treatment of epilepsy. This guidance, published in October 2004, specifically covers the importance of informing patients and their families of the risks of sudden death in epilepsy. In addition, we have made a grant of almost £290,000 to the National Society for Epilepsy to improve support and information for people with epilepsy and their families.

Details of the sites and diagnostic tests piloted are shown in the list. Pilot diagnostics data collections have helped refine the list of diagnostic tests to be included in the national data collection for the 18-week target. The Department meets the service costs of research commissioned by its research partners that takes place in the national health service. The funding allocated for this purpose supports a substantial body of health research, including Cancer Research UK's trial of the MCM5 test for bladder and prostate cancer.The trial at the Freeman hospital in Newcastle is expected to involve 3,000 patients and is not due to finish until 2006. At this stage it is not possible to speculate on when the test might be introduced on a regular basis into the NHS as the results from the trial will require in depth analysis.

Ambulance emergency medical technician and paramedic qualifying training includes recognition and treatment of poisoning, including reference to carbon monoxide. It is also covered by the clinical practice guidelines for use in United Kingdom ambulance services, produced by the Joint Royal Colleges Ambulance Liaison Committee.

[holding answer 17 October 2005]: The new diagnostic data collection will start across England in January 2006. It will enable the national health service to track progress towards meeting the Government's election pledge of a maximum wait of no more than 18 weeks from general practitioner referral to the start of hospital treatment. It will identify areas requiring focused attention and assist diagnostics capacity planning for future years.The Department estimates that, once the initial implementation stage is complete and being submitted through Unify, the Department's new system designed to capture and analyse NHS performance data, the cost burden on the NHS will be lower than for existing monthly returns.
