The Railways and Transport Safety Act 2003 amends the Road Traffic Act 1988 to authorise the police to conduct compulsory roadside test for impairment and for the presence of drugs. Any equipment used to test for drugs by testing a specimen of sweat or saliva must be of a type approved by the Secretary of State. Since there is as yet no such equipment, tests cannot at present be conducted. It is therefore not possible to give the unit cost per test or the cost of the equipment necessary to conduct the test.A detailed device specification is currently in preparation. We hope this will be available in early 2006. Manufacturers will then be able to submit to the rigorous type approval process any device they have developed which complies with the specification.

New community pharmacy contractual arrangements provide the framework for pharmacy to make a positive contribution to tackling key health priorities such as obesity and coronary heart disease and to improving the health of the public, including where health inequalities persist.Some pharmacies offer blood pressure testing, cholesterol testing or a range of other screening tests. Such testing should be in accordance with relevant national screening guidelines, where they exist, and in line with local primary care trust (PCT) programmes for prevention and management of risk factors and with robust quality assurance processes.We published ““Choosing Health through Pharmacy— A programme for pharmaceutical public health”” in April 2005, which aims to maximise the contribution of pharmacists, their staff and the premises in which they work to improve health and reduce health inequalities. This strategy will enable pharmacists and their staff to make better use of their skills and it will open up opportunities for pharmacy to make a bigger difference to improving the health of people in England. PCTs can commission local enhanced services to meet identified health needs.Although we do not hold details centrally on the number of pharmacists planning to offer health checks or the numbers of such checks, we have developed an assurance framework for PCTs to use in monitoring pharmacy's implementation of new contractual requirements, and also strategic tests for strategic health authorities to monitor how PCTs are using the new framework to deliver key health priorities and objectives locally.

[holding answer 8 December 2005]: Latest performance data from local delivery plan monitoring shows that at September 2005 57.3 per cent. of people with diabetes had received screening for diabetic retinopathy over the past 12 months. National figures are not yet available on the numbers of patients offered screening. However, it is expected that at least 70 per cent. of people offered screening will receive it.2004–05 data from general practices participating in the quality outcomes framework of the new general medical services contract records 85 per cent. of people with diabetes as screened. These two sources produce different information because the two data collections have different purposes and definitions.

Quality assurance standards have been developed and piloted by the UK National Screening Committee's national screening programme for sight threatening retinopathy. A full list of the quality assurance standards can be found on the National Screening Programme's website at: www.nscretinopathy.org.uk. Screening programme managers are responsible for ensuring that programmes meet overall quality requirements. Strategic health authorities are responsible for ensuring action is taken on quality assurance recommendations.

Since December 2003, in vitro diagnostic devices placed on the United Kingdom market have to conform to the requirements of the In Vitro Diagnostic Devices Directive (98/79/EEC) which has been transposed into UK law by the Medical Devices Regulations 2002 (SI2002 No.618).In vitro diagnostic test kits for hepatitis C attract the highest level of control and need to have their compliance with the directive assessed by an independent certification body known as a Notified Body. The manufacturer also needs to register with the regulatory authority in the member state where the device is to be placed on the market.In so far as can be determined, no such registrations have been made for this type of test kit with the Medicines and Healthcare products Regulatory Agency in the UK, and we are not aware of any hepatitis C home test kits that are in development.

The Department has published information and guidance for health professionals and for the public, highlighting those considered to be at risk of hepatitis C infection and who should consider being tested, or should be offered testing, for hepatitis C infection. This forms part of the hepatitis C awareness campaign and there is currently a continuing programme to raise awareness of hepatitis C among the public and those groups at risk.In addition, a new national health service hepatitis C awareness website was launched and has been published at www.hepc.nhs.uk and a new national hepatitis C freephone information line at 0800 451451.

All women with suspected breast cancer should be referred urgently by their general practitioner under the two week out-patient waiting time standard. From the data centrally collected, we cannot identify those women who were routinely referred with other breast conditions but who were subsequently diagnosed with breast cancer. The number of patients who were treated for breast cancer who were referred under the two week wait and those who were referred through other routes is shown in the table.
The Cancer Plan target of 31 days from diagnosis with cancer to first treatment, to be achieved from December 2005, picks up those patients who are referred with other conditions but who are subsequently diagnosed with cancer to ensure they receive prompt treatment.

The Health Protection Agency (HPA) has carried out surveys of the prevalence of hepatitis C virus (HCV) antibodies in several populations that have differing degrees of risk of acquiring HCV. These are: adults in hospitals, women attending antenatal clinics, people attending genitourinary medicine (GUM) clinics, prisoners, and injecting drug users (IDUs). These and the areas where the surveys were undertaken are outlined in the table¹.

Primary care trusts will receive funding for implementing the targets in the Choosing Health White Paper in their mainstream allocations and we will be monitoring the outcomes from this investment. In particular, the progress towards targets to reduce the level of new infections of gonorrhoea, to ensure patients have access to genitor-urinary medicine clinics within 48 hours by 2008 and numbers of screens undertaken in the Chlamydia screening programme. These improved performance measures, should significantly strengthen the incentive for local investment and service modernisation.In addition, a letter has been sent by the Department to strategic health authority chief executives highlighting that when considering any savings from implementing ““Commissioning a patient led NHS””, savings should not be identified from those posts working on implementation of Choosing Health. This includes posts in frontline services.

Primary care trusts are responsible for implementing diabetic retinopathy screening to national standards as set out in ““Improvement, Expansion and Reform: the next three years””, the planning and performance framework for 2003 to 2006 and diabetes national service framework delivery strategy. It is for local health services to decide how they want the programme to be delivered locally. Optometrists have played an important role on the national diabetic retinopathy screening project advisory group set up to steer the development of the programme.

To improve the accuracy of mammography screening we have upgraded the national health service breast screening programme (NHSBSP) by offering two-view mammography, two x-rays of each breast, at all screening rounds. Over 95 per cent. of breast screening units have now implemented two-views, resulting in a 31 per cent. increase in the number of cancers detected annually since April 2001, when roll out began.The quality assurance process ensures that the high quality standards of the NHSBSP are not only maintained, but continue to rise year on year. As part of this continuous improvement to the service, the NHSBSP, with the Department and Cancer Research UK funding, is about to begin a major study looking at the use of computer aided reading of mammograms in addition to a human reader. This study is expected to report in two years time.The Department is also funding, along with the Medical Research Council and Cancer Research UK, the UK Coordinating Committee on Cancer Research randomised controlled trial of the effect on breast cancer mortality of annual mammographic screening of women starting at age 40, the age trial. The main aim of the study is to evaluate the effect of annual mammographic screening of women starting at ages 40 to 41 on mortality from breast cancer, thus giving a definitive answer to the outstanding question of whether population screening below 50 is beneficial or not. As the trial is primarily looking at mortality benefits, full results are not expected before spring 2006.
