Mrs. Calton: To ask the Secretary of State for Health what assessment he has made of waiting times for (a) stairlifts and (b) downstairs extensions and modifications when people have been assessed as unable to climb stairs or to use standard toilet or washing facilities; and if he will make a statement. [133484] Phil Hope: I have been asked to reply. Housing adaptations for disabled people such as stair lifts, level access showers and downstairs extensions can be grant aided under the disabled facilities grant programme. This is a mandatory grant available to eligible applicants in order to help them to continue to live independently in their own homes. It is funded by the Office of the Deputy Prime Minister and administered through local authorities. The Office of the Deputy Prime Minister's budget for this grant has been increased substantially to £100 million for this year. However demand for the grant remains high and waiting lists do occur. Local authorities are required to respond to any full application for a disabled facilities grant within six months. The Government do not collect information on the length of any waiting lists. However jointly with the Department of Health we have issued draft guidance to local authorities setting out best practice in delivering this service. This includes target times for the complete delivery process including needs assessment, processing of the application and the completion of building works. These target times will depend on the complexity of, and priority attached to, individual cases but the guidance suggests that the maximum target time for the entire process should in any event be no more than 260 working days and only 80 working days in high priority cases. We are currently considering responses to this consultation and hope to issue final guidance to local housing and social service authorities before the end of this year.

Mrs. Calton: To ask the Secretary of State for Health (1) whether his Department has completed an audit of whether the guidance "Good Practice in Continence Services" has improved paediatric continence services; [132876] (2) what assessment his Department has made of how the children's national service framework will improve services for children with continence problems; [132885] (3) what assessment his Department has made of how many children in England suffer from (a) nocturnal enuresis, (b) daytime wetting and (c) constipation and soiling. [132887] Dr. Ladyman: Good Practice in Continence Services was issued by the Department in April 2000 and provides guidance to primary care trusts (PCTs) on continence services. This includes that PCTs should have specialist continence services in place, which provide patients with an individual assessment of their needs. Provision of these services in England is a matter for PCTs, which are responsible for determining the level of services required to meet the needs of their local populations. The children's national service framework (NSF) will set out standards covering a broad range of services, including the management of common childhood conditions. Whilst few specific conditions will be covered, the standards are likely to cover what support should be available to children and their parents in managing a wide range of conditions and problems, including enuresis and soiling. The NSF will emphasise the promotion of evidence-based clinical guidelines and a number of examples of good practice in a range of different areas. Information on the prevalence of these common conditions in childhood is not collected centrally.

Mrs. Calton: To ask the Secretary of State for the Home Department pursuant to the answer of 7 July 2003, Official Report, column 647W, on robbery/ burglary victims, how many injuries were sustained in muggings and robberies in each year. [125796] Ms Blears: Data on the number of injuries sustained in muggings and robberies in the specified years is not published. However, the British Crime Survey (BCS) is able to provide trend estimates of the number of incidents of mugging and robbery in England and Wales (Table A), additionally the BCS also collects information on the proportion of violent incidents involving injury (Table B). Violent incidents from the BCS can be classified into a violence typology comprising domestic, mugging, stranger and acquaintance incidents. Violent crime from the BCS can also be separated into robbery, wounding, common assault, and snatch theft. The information regarding injuries sustained for this latter breakdown is not published prior to 2001-02 BCS interviews. Mugging is a popular rather than a legal term, comprising robbery, attempted robbery and snatch theft from the person. The BCS does not cover incidents against those aged under 16, nor those not living in private households. Mrs. Calton: To ask the Secretary of State for Health what funding was available for research into a cure for myeloma in each of the last five years; and if he will make a statement about future funding. [122028] Miss Melanie Johnson: The main Government agency for research into the causes of and treatments for disease is the Medical Research Council (MRC) which receives its funding from the Department of Trade and Industry via the Office for Science and Technology. The MRCs expenditure on research on myeloma is shown in the table. _________________________________________________________________________________________________. The MRC has also supported two major myeloma studies: Myeloma VII and Myeloma VIII. Because the MRC has changed the way in which information is kept about funding of trials, the figures in the table do not include spend on these two studies. The MRC has recently awarded £1 million to the Myeloma IX trial involving 1,600 patients. This trial will evaluate a number of different therapies and modalities of treatment used and being developed for myeloma, to provide information on their impact on response rates, survival and quality of life. The MRC funds a considerable amount of basic underpinning research which is excluded from site- specific figures. As a guide, the MRC spent £75 million on cancer research in 2001-02. In addition, research classified as undertaken in one site may well have implications in another--leukaemia would be a relevant example in the case of myeloma. The Department of Health meets the national health service support costs of research funded by the MRC and research charities. In 2000-01, an estimated £73.2 million was spent on cancer research through research and development allocations to NHS organisations. Management of the research supported by these allocations is devolved and expenditure at project level is not held centrally by the Department.

[121064] Miss Melanie Johnson: Figures for all years since 1980 are not available but the table shows estimates from a population survey, the Health Survey for England, from 1991, when the survey began. Although earlier surveys have information on overweight and obesity comparisons are difficult because of different survey methodologies. The Health Survey covers ages 16 and over. Mrs. Calton: To ask the Secretary of State for Culture, Media and Sport what measures will be put in place to ensure that licences issued under the terms of the Licensing Bill by one local authority will be recognised as valid by other local authorities. [119868] Mr. Caborn: Personal licences must be issued by all licensing authorities in accordance with the provision of the Bill. Further, regulations made by the Secretary of State will prescribe a standard form for a personal licence. No discretion is given to a licensing authority to reject the validity of a personal licence properly issued by any licensing authority. The Government are also committed to setting up a central licensing register, which would allow licensing authorities to check the issue of a personal licence with another licensing authority. Work with local authorities and other key partners to set up such a database is in hand and we will drive the work forward in parallel with the Bill to ensure that a database is available at the earliest opportunity. A premises licence is also granted in accordance with the provisions of the Bill by the licensing authority in whose area the premises is situated. The form of a premises licence will also be prescribed by the Secretary of State in regulations. The issue of recognition of the validity of a premises licence by another licensing authority may only arise when premises straddle the area of more than one authority. If the other authority made relevant representations on an application for a premises licence, and is aggrieved by the licensing authorities decision, it may appeal that decision to the magistrate's court.

Mrs. Calton: To ask the Secretary of State for the Home Department (1) what plans he has to reduce the number of women in prison for non-violent offences; [119980] (2) what plans he has to reduce re-offending among women released from prison; [119981] (3) whether it is his policy to reduce the female prison population; [119982] (4) what assessment has been made of the scope for the awarding of non-custodial sentences for women who are mothers. [120019] Paul Goggins: The significant increase in the female prison population, and the wider consequences of this in terms of disruption to their families, particularly their children, underlines the importance of responding specifically to the particular needs and characteristics of women offenders. We are taking forward the Women's Offending Reduction Programme over the next three years in order to promote a more distinct response to the range of factors that have an impact on why women offend, and encourages joint working between departments, agencies and other relevant organisations. A primary focus will be on improving community based interventions that are better tailored to the needs of women, and encouraging greater use of community disposals to ensure that custody is only used for women offenders who really need to be there. It is, of course, for the courts to decide what sentence is appropriate in individual cases, but we intend to support and encourage greater use of community sentences for women by making sure that there is a comprehensive package of community interventions and services to meet their particular needs, including child care facilities, safe housing, drug treatment or mental health services. We will continue to ensure that the courts are made fully aware of the community options that are available. We are also introducing new sentencing powers in the Criminal Justice Bill that will allow offenders on short custodial sentences to focus their rehabilitative work in the community, enabling them to maintain family ties and employment. The Criminal Justice Bill will introduce `Custody minus', under which offenders will have a custodial sentence of under 12 months suspended providing they follow set requirements in the community, breach of which would return them to prison. There is a new sentence of intermittent custody, under which offenders can spend part of the week in custody and part `on licence' in the community. One of the pilots for this sentence will be at a women's prison, Her Majesty's Prison Morton Hall. Intermittent Custody will provide an alternative to full-time custody for suitable offenders and will enable them to serve their sentence in a way which dovetails more closely with their personal circumstances. For women who do need to be held in custody, resettlement is a vital element of the Women's Offending Reduction Programme. More effective re-integration into the community for women prisoners on release should have a positive impact on re-offending rates. The implementation of the Women's Estate resettlement strategy will ensure that women are kept as close to home as possible, that good family ties are maintained to minimise the impact on children separated from their mothers, and necessary local community links are made to meet the range of women's resettlement needs.

[119675] Ms Rosie Winterton: The National Institute for Clinical Excellence (NICE) issued guidance on the use of newer (atypical) antipsychotic drugs for the treatment of schizophrenia in June 2002. It is recommended that atypical antipsychotics should be considered as options for the treatment of schizophrenia. The clinician responsible for prescribing the medication should do so following appropriate assessment of the patient and taking into account the views of the patient (or their advocate if appropriate), and the relative benefits and side effects. The statistics currently available on the use of atypical antipsychotics indicate a continuing rise in the prescribing of these products, but no estimates are available of the number of patients who may not be receiving treatment in accordance with NICE guidance. National health service bodies are under a statutory obligation to fund treatments recommended in NICE technology appraisals. We expect primary care trusts (PCTs) to meet their statutory obligations, and strategic health authorities to follow up any allegations of non- compliance. We last reminded PCTs of their obligations in guidance published in January 2003.

[119689] Mr. Hutton: The Department does not collect this information centrally. However, we do know that one third of all reported injuries that result in three or more days off work are caused by manual handling accidents and that manual handling accidents and back pain account for approximately 40 per cent. of all national health service sickness absence. Evidence suggests that the introduction of lifting equipment for patients, following suitable risk assessment, has a greater impact on the number of manual handling accidents to both staff and patients than simply increasing staff numbers. We believe that the current strategy of providing increased levels of mechanical aids is responsible for the reduction in manual handing accidents reported by 33 per cent. of trusts in the National Audit Office's recent report "A Safer Place to Work--Improving the Management of Health and Safety Risks to Staff in NHS Trusts". The Manual Handling Operations Regulations 1992 are not specific to the NHS. However, the Health and Safety Executive has issued non-statutory guidance, "Manual Handling in the Health Services", a copy of which will be placed in the Library.

Mrs. Calton: To ask the Secretary of State for Health if he will make a statement on compliance within the NHS with the NHS Manual Handling Regulations. [119690] Mr. Hutton: The Manual Handling Operations Regulations 1992 (as amended) apply to all work activities and are the responsibility of the Department of Work and Pensions. The National audit Office report, "A Safer Place to Work--Improving the Management of Health and Safety Risks to Staff in NHS Trusts", showed a reduction of 33 per cent. in manual handling accidents over the last three years. The Department is reviewing approaches taken by national health service trusts in evaluating compliance with the current guidance and will be considering the need for a more consistent approach to evaluation. The Health and Safety Executive (HSE) enforces the above regulations in NHS trusts and other healthcare premises and assesses compliance with the regulations as a key part of most trust inspections. I am informed that the HSE has not prosecuted any NHS trusts for breaches of the regulations, but it has served a number of individual trusts with statutory notices requiring them to improve compliance with various requirements of the regulations. [119947] Miss Melanie Johnson: My right hon. Friend, the Prime Minister's Strategy Unit, in close collaboration with other Government Departments, including the Department of Health, is currently developing the national alcohol harm reduction strategy, which will sit alongside the national drugs strategy. The Strategy Unit is due to publish an interim analysis of the harms associated with alcohol misuse in the summer. The final report, which will set out the cross-Governmental alcohol harm reduction strategy, is planned for publication in the autumn. As part of this project the Strategy Unit and the then Under-Secretary of State for Health, my hon. Friend the Member for Salford (Ms Blears), have had on-going discussions with all relevant stakeholders, including the alcoholic drinks industry. There is currently no specific maximum waiting time target for treatment for those with alcohol dependency. However the final report will consider a wide variety of issues around treatment and support for those dependent on alcohol, and will then move to consider specific policy issues, including unit and warning labelling. Provision of appropriate health service provision is the responsibility of local commissioners of services based on assessment of need. There is a wide range of services available for those with alcohol problems requiring treatment. Medical management is available in primary and secondary care. Specialist services provide a range of psychotherapeutic interventions focusing on alcohol use and the wider contributory factors to development of dependence.

[119780] Dr. Ladyman: A new expert group of the Committee on Safety of Medicines (CSM) has been convened to further review the safety of selective serotonin re-uptake inhibitors (SSRIs) and to ensure the advice in the product information for both patients and prescribers is optimal for the safe use of these products. This group has been asked to examine what implications, if any, the CSM's recent advice on the use of Seroxat in children have for the use of Seroxat in adults and for other SSRIs. The timetable for the group's programme of work has been planned with a view to the availability of advice as promptly as possible, commensurate with a full and detailed consideration of the evidence including the experiences of patients. As part of its initial considerations, the group has advised on strengthened advice on side effects and withdrawal reactions for inclusion in the patient information leaflet on Seroxat. Product information for other SSRIs will also be considered. The advice of the CSM, most recently in 2001, is that the benefits of Seroxat, when used in adults for licensed indications, outweigh the risks. Regulatory authorities world-wide have also taken a similar position. Depression and anxiety disorders are serious conditions that can cause severe suffering and are associated with an increased risk of suicide. The availability of effective treatments is critically important. In the absence of evidence indicating that the risks of using Seroxat in adults outweigh the benefits, it would be difficult to justify withdrawing the medicine from use. The Yellow Card Scheme is recognised to be one of the best in the world in terms of the level of reporting, has a proven track record of identifying new drug safety hazards and enables the monitoring of all medicines, including Seroxat, in clinical use. The effectiveness of the Scheme is under continuous review by the Medicines and Healthcare products Regulatory Agency and the CSM. From April 2003, the first phase of patient reporting of suspected adverse drug reactions via NHS Direct was introduced. When fully rolled out in England and Wales, this will enable the first hand experiences of patients to be utilised in the identification and assessment of drug safety issues. Other measures to further strengthen the Yellow Card Scheme include a new regional monitoring centre, established in Scotland last year and the inclusion of education on yellow card reporting in the training of nurse prescribers.

Mrs. Calton: To ask the Secretary of State for Health what measures are in place to provide a multi-agency approach to treatment for those with alcohol dependency who also have mental health and other drug problems. [119906] Miss Melanie Johnson: The Department of Health issued guidance last year on the management and delivery of effective treatment for adults with severe mental illness and co-existing drug or alcohol misuse problems. The guidance highlights the responsibilities of drug action teams and local implementation teams to work together locally in order to implement the guidance in accordance with local needs. The National Treatment Agency for substance misuse has also issued guidance in the last year for commissioners, `Models of Care'. The framework of this guidance applies to alcohol and drug treatment services and strongly supports the development of multi-agency approaches to treatment based on comprehensive assessment of needs, including for those with dual diagnoses. In addition, the National Institute for Mental Health in England is also working to identify how access to effective treatment can be improved and waiting times reduced. Many of the services affected by this work are joint drug and alcohol services. The Department of Health is preparing a new strategy on alcohol harm reduction which will set out a coherent and practical framework for a responsible approach which reinforces the message about sensible drinking and seeks to tackle alcohol misuse in all its forms. The NHS Plan said that we will be implementing the strategy by 2004, and we are on course to achieve that target. The Cabinet Office strategy unit is to undertake a project on alcohol misuse. Conclusions from the project will form the basis of the Government's alcohol strategy.

Mrs. Calton: To ask the Secretary of State for Health what information, preparation and guidance have been provided to tertiary centres on infectious and communicable diseases in the event of a biological attack. [99351] Mr. Hutton: Comprehensive guidance, to ensure that the National Health Service is prepared to respond to major incidents or terrorist attacks, was first issued in November 1998, and was entitled Planning for major incidents: the NHS guidance. The Health Protection Agency (HPA) (formerly the Public Health Laboratory Service (PHLS)) has for many years undertaken surveillance of biological agents on the Department's behalf. The approach taken has been to build on the public health need to have in place clinical and laboratory based surveillance for the most likely agents that could be used in a biological attack. This work resulted in the document published by the Department in March 2000, Deliberate release of biological and chemical agents. In October 2001 a further package of guidance on the deliberate release of anthrax, smallpox, plague, botulism and the approach to dealing with a chemical, biological or radiological deliberate release was issued. Complementary operational guidance for clinicians, including general practitioners (GPs), was placed on the then PHLS's (now the HPA's) website. These are regularly updated. In December 2002, the Department published Interim guidelines for smallpox response and management in the post eradication era on the departmental website. All such guidance is available on the Department's website at www.doh.gov.uk/epcu/cbr/intro.htm with links to complementary guidance on the HPA's website. The information and guidance provided by the Department to the NHS is, of course, accessible to the whole NHS.

Mrs. Calton: To ask the Secretary of State for Health (1) what progress have been made in reducing energy use in the NHS; [108683] (2) what progress has been made on embedding sustainable practice in the National Health Service; [108652] (3) what progress have been made in producing a low carbon economy in the NHS. [108653] Mr. Hutton: The National Health Service has been highly successful in meeting the energy efficiency target of 20 per cent, reduction in energy use from 1990 to March 2000. In April 2001, in accordance with the Government Climate Change Programme, the Department of Health introduced mandatory targets for NHS organisations in England with the aim or reducing the level of primary energy consumed and also carbon emissions produced by the NHS. The targets are: To reduce the level of primary energy consumption by 15 per cent, or 0.15 million tonnes carbon from March 2000 to March 2010. To achieve a target of 32--55 Gigajoules per 100 cubic metres (Gj/lOOcu.m) energy efficiency performance for the healthcare estate for all new capital developments andmajor redevelopments or refurbishments; and that all existing facilities should achieve a target of 55-65 Gj/lOOcu.m. The Department does not differentiate between energy efficiency and carbon efficiency targets. To assist the NHS work towards meeting these targets and embedding sustainable practices in the NHS, NHS Estates issued the "New Environmental Strategy for the NHS" in 2002 together with the guidance document, "Sustainable Development in the NHS" and the software tool, "NHS Environmental Assessment Tool (NEAT)". NHS Estates is also working closely with Action Energy and the Carbon Trust to support the NHS and provide them with best practice advice and guidance on sustainable practices and improving energy efficiency.
