My Lords, it is difficult for me to answer that. Letters from doctors are required for many benefits. I am sure that the noble Lord is referring particularly to incapacity benefit, which is an issue. He will know that we will shortly be bringing forward a Green Paper on welfare reform, part of which will encompass the relationship with GPs and the health service because, clearly, they have an important role to play.

Anyone who is a ““relevant foreign national”” as defined in Part 10 of the Immigration Rules should normally be required to register with the police. The term ““relevant foreign national”” applies to anyone who is aged 16 or over and is the national or citizen of a country or territory listed in Appendix 2 to the rules, or is stateless, or holds a non-national travel document. The Immigration Rules can be found on the Immigration and Nationality Directorate website (www.ind.homeoffice.gov.uk) in the ““Law and Policy”” section.

Like anyone else who is subject to immigration control, a person who is a ““relevant foreign national”” will be given an entry clearance if he or she meets the relevant criteria as set out in the Immigration Rules, and there are no overriding reasons for refusing entry. By ““overriding reasons””, I mean the general grounds for refusing leave to enter which are set out in Part 9 of the rules.

The Government have given a clear commitment in the White Paper Fairness For All and elsewhere that the Commission for Equality and Human Rights will provide continued support for the local race equality work carried out by the network of racial equality councils and other bodies; and that the CEHR will maintain funding for local race equality work at current levels for the foreseeable future. Promoting race equality and good community relations will be at the heart of the CEHR’s work and will build on the CRE’s achievements and its partnership with RECs. The CEHR’s powers will give it a strengthened good relations role, covering not only the promotion of good relations but combating hate crime and working to eliminate prejudice, hatred and hostility. The CEHR’s regional arrangements throughout Great Britain will enable it to work more closely with RECs and communities in their local settings, understanding local and regional priorities and delivering services that are relevant and appropriate.

My Lords, we are consulting on a reduction in the number of strategic health authorities and possible changes in the boundaries of PCTs. We committed ourselves to saving £250 million in this area in our manifesto and we will move forward on that. That will be more money for front-line services. The PCT changes will also strengthen commissioning in the NHS, as has recently been made clear in a number of ministerial statements to that effect.

My Lords, is the Minister aware that primary care trusts are now worried about payment by results because the projected increase in payments to hospitals, which comes from primary care trusts, will further increase if hospitals increase their activity? My Lords, payment by results is being introduced over a four-year transition period, during which the year-on-year income changes for baseline activity will be capped at 2 per cent for individual providers, so there will be a limit on how much income people can use. The noble Lord asked what PCTs should be doing. They should pursue stronger commissioning in how they take forward their agenda and incentivise practice-based commissioning so that GPs are more in the driving seat.

The requirement to register with the police once they have arrived in the United Kingdom is a condition of the entry clearance issued by the British Embassy. Chinese nationals are ““relevant foreign nationals”” for the purposes of Part 10 of the Immigration Rules. These state that a ““relevant foreign national”” aged 16 or over who is given limited leave to enter the United Kingdom for longer than six months should normally be required to register with the police unless he is given leave in one of the categories listed in paragraph 326(2) of the rules. Students are not one of the exempt categories. This is not a new requirement, nor is it confined to Chinese nationals.

There are means at every level—for example, when patients are given assistance and advice. Is that advice being made available to patients? Also, how strong is public and community participation in the care of patients in terms of trusts? What is the process and to what extent does the Department of Health encourage a more open policy with patients?

My Lords, a good deal of work has gone into improving the process at local level. We now have, as a result of the 2003 changes, patient advice and liaison services—or PALS—which are playing a big part in advising people and helping them to address their concerns on the spot at the local level. Trusts are being more outward going. There is a cultural change of less defensiveness, which shows up here in the sense that people are resolving these complaints at the local level. It demonstrates that the Healthcare Commission itself is encouraging that local resolution.

My Lords, I am sure that my colleagues in the Government who meet their Chinese counterparts will be discussing such issues. However, I am glad to say that China is one of the 89 countries that have ratified the framework convention. Of course, ratifying a convention and implementing legislation are two very different things, but that is the first sign that China has the political will to do something about tobacco smoking.

My Lords, it will be central to the new children’s trusts because local authorities will be taking forward the work on national minimum allowances and on the other measures we have set out to encourage the greater recruitment of foster carers, as well as better training and support for them.

My Lords, GPs have a vital role to play in the advice and encouragement that they give their patients in relation to considering work and the health benefits of going back to work. I understand the noble Lord's point about paperwork. I have taken part in a number of good discussions with the BMA and other doctor groups, and it is a matter that we are fully considering.

The Government received 83 written responses to their consultation exercise to establish an Inspectorate for Justice and Community Safety. Of those, 40 supported the proposals, 29 were against and 14 did not make clear their preference.
